摘 要:目的探讨血清25羟维生素D[25(OH)D]水平与慢性肾脏病(CKD)患者左心室肥厚(LVH)的相关性。方法选取南阳市第一人民医院2019年1月至2020年8月收治的126例CKD患者作为观察组,选取同期100例健康体检者作为对照组。比较两组患者左心室质量指数(LVMI)、血清25(OH)D水平,分析LVMI、血清25(OH)D水平与疾病严重程度的相关性,分析血清25(OH)D水平与LVMI的相关性以及血清25(OH)D检测对LVH的诊断价值。结果观察组LVMI高于对照组,血清25(OH)D水平低于对照组(P<0.05);随着病情加重,LVMI逐渐升高,血清25(OH)D水平逐渐降低(P<0.05)。LVMI与疾病严重程度呈正相关,血清25(OH)D水平与疾病严重程度呈负相关(P<0.05),血清25(OH)D水平与LVMI呈负相关(P<0.05)。受试者工作特征(ROC)曲线显示,血清25(OH)D诊断LVH的曲线下面积(AUC)为0.718。结论CKD患者LVMI、血清25(OH)D水平与疾病严重程度密切相关。血清25(OH)D与LVH的发生密切相关,临床可通过检测其水平判断LVH严重程度,为临床采取针对性治疗提供参考。Objective To explore the association between serum 25-hydroxyvitamin D[25(OH)D]levels and left ventricular hypertrophy(LVH)in patients with chronic kidney disease(CKD).Methods A total of 126 CKD patients in Nanyang First People’s Hospital from January 2019 to August 2020 were selected as observation group,and 100 healthy subjects in the same period were selected as control group.Left ventricular mass index(LVMI)and serum 25(OH)D level were compared between the two groups.LVMI and serum 25(OH)D level in patients with different stages of CKD were compared.Analyze the correlation between LVMI level,serum 25(OH)D level and the degree of disease,and the diagnostic value of serum 25(OH)D detection for LVH.Results The LVMI in observation group was higher than that in control group,and the level of serum 25(OH)D was lower than that in control group(P<0.05).With the exacerbation of the disease,LVMI gradually increased,and serum 25(OH)D level gradually decreased(P<0.05).LVMI was positively correlated with the degree of disease,while 25(OH)D was negatively correlated with the degree of disease(P<0.05).Serum 25(OH)D was negatively correlated with LVMI(P<0.05).The receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of serum 25(OH)D in diagnosis of LVH was 0.718.Conclusions Serum 25(OH)D and LVMI levels in CKD patients are closely related to the severity of the disease.Serum 25(OH)D has a certain relationship with the occurrence of LVH.LVH could be determined by detecting the level of serum 25(OH)D,which provides reference for the clinical targeted treatment.
