摘　　要：目的:为医疗卫生机构医院信息系统(HIS)药品字典管理和国家药品配备使用监测提供意见和建议。方法:对许昌市参与2019年公立医疗卫生机构药品配备使用情况监测分析工作的30所医疗卫生机构HIS供应服务商、药品字典数据项(字段)选取、管理制度建设和数据规范性等情况进行调查。结果:调查显示,(1)30所医疗卫生机构使用的HIS来自12家供应服务商。部分医疗卫生机构未在药品字典中选取批准文号、生产企业和包装材质等关键信息作为数据项(字段)。(2)30所医疗卫生机构均未制订单独的药品字典相关管理制度。不同医疗卫生机构药品字典维护人员的岗位不同,其中有1/3的维护人员为非药学专业技术人员。30所医疗卫生机构均在不同程度上存在药品基础数据维护不规范的情况。结论:30所医疗卫生机构的药品字典建设不标准、不统一,管理水平参差不齐。建议卫生行政部门制定药品字典建设标准和规范,推动医疗卫生机构药品字典建设标准化。同时,各医疗卫生机构应建立健全药品字典相关管理制度,明确药品字典维护人员任职条件,提高药品基础数据的准确性和规范性,以实现药品数据的互联互通,为国家有关决策的制订提供可靠依据。Objective: To provide opinions and suggestions for Hospital Information System(HIS) drug dictionary management and medicine supply and utilization monitoring within national public medical and health institutions(MHIs). Methods: An investigation was conducted about HIS supplier service suppliers,drug dictionary data item(field) selection, management system construction, data standardization of 30 health institutions which participated in the monitoring of medicine supply and utilization within MHIs in Xuchang in 2019. Results:(1)The HIS are used by 30 MHIs from 12 suppliers. Some MHIs don’t select key information such as approval number, manufacturer and packaging material as data items(fields) in drug dictionary.(2)All 30 MHIs have not established a specialized drug dictionary management system. Positions of the drug dictionary maintenance personnel are different. Among them, 1/3 of the maintenance personnel are not pharmaceutical technicians. Incomplete maintenance of basic drug information occurs in all the 30 MHIs. Conclusion: The constructions of drug dictionaries of the 30 MHIs are not standard and uniform. The management level is uneven.It is recommended that health administrative department establish the drug dictionary management standard and specification, promote the standardization of drug dictionary construction in MHIs. Meanwhile, MHIs should establish and improve the drug dictionary management system, clarify the requirements for drug dictionary maintenance personnel, improve the accuracy and standardization of drug basic information maintenance to realize the interconnection of drug data and provide a reliable basis for the relevant decisions.
