摘 要:目的探讨患者参与的饮食管理对肝移植术后患者胃肠功能恢复的影响。方法选取2018年1月-2019年1月在郑州某三级甲等医院行肝移植的84例患者为研究对象,将其随机分为对照组和观察组,对照组给予常规饮食指导,观察组给予患者参与的饮食管理模式,比较2组患者术后的胃肠功能恢复情况、相关并发症发生率,并采用患者营养主观评估量表(patient-generated subjective global assessment,PG-SGA)评价患者术后1周、出院4周和3个月的营养状况。结果干预后,观察组患者术后7 d胃肠道补给量多于对照组(t=15.025,P=0.021),术后感染发生率低于对照组(χ^(2)=9.355,P=0.002);2组PG-SGA评分在时间、组间及交互效应上比较,差异均有统计学意义(F时间=46.215,P<0.001;F组间=53.845,P<0.001;F交互=66.914,P<0.001)。结论对肝移植患者实施患者参与的饮食管理模式,能够促进患者胃肠功能恢复,改善患者营养状况,降低相关并发症的发生率。Objective To investigate the effect of the patient participation-based dietary intervention(PPDI)on postoperative gastrointestinal function recovery of patients undergoing the liver transplantation.Methods Eighty-four patients who underwent liver transplantation in our hospital between January 2018 and January 2019 were randomly divided into a control group and an observation group.The control group was given routine dietary guide,while the observation group received patient participation-based dietary intervention.The recovery of gastrointestinal function and the incidence of complications were compared between the two groups.The nutritional status were evaluated using the patient-generated subjective global assessment(PG-SGA)one week after the surgery,as well as 4 weeks and 3 months after discharge.Results Compared with the control group,the gastrointestinal tract replenishment of the observation group increased significantly 7 days after the surgery(t=15.025,P=0.021),while the incidence of postoperative infection decreased significantly(χ^(2)=9.355,P=0.002).Moreover,significant differences were observed in the PG-SGA score in the time,intergroup and interaction effects(Ftime=46.215,P<0.001;Fintergroup=53.845,P<0.001;Finteraction=66.914,P<0.001).Conclusion The patient participation-based dietary intervention can promote the recovery of the gastrointestinal function,reduce the incidence of related complications and improve the nutritional status of patients after their liver transplantation.
