SCORTEN评分对SJS-TEN住院周期及预后的评估价值

摘　　要：目的回顾性分析中毒性表皮坏死松解症(TEN)临床结局的严重程度(SCORTEN)评分在史蒂文斯-约翰逊综合征及中毒性表皮坏死松解症(SJS-TEN)住院周期及预后中的应用价值。方法对2015年1月—2019年12月在我科住院的34例SJS-TEN患者住院第1天和第3天分别进行预测SCORTEN评分,并分析评分和重症监护室(ICU)住院时间、总住院时间、预后之间的关系。结果第1天SCORTEN评分和ICU住院天数(r=0.505,P<0.05)及总住院天数(r=0.530,P<0.05)均呈明显正相关;第3天SCORTEN评分和ICU住院天数(r=0.543,P<0.05)呈明显正相关;第1天和第3天心率>120次/min、皮肤剥脱面积>10%体表总面积(TBSA)的患者患者住院总时间明显延长(P<0.05);第1天HCO3-<20 mmol/L的患者住院总时间明显延长(P<0.001)。结论第1天和第3天SCORTEN评分越高,提示患者病情越重,ICU住院时间及住院总时间越长。Objective In order to assess the evaluation of score of toxic epidermal necrosis(SCORTEN)score on Stevens-Johnson syndrome(SJS)/toxic epidermal necrosis(TEN)hospital stay and prognosis by retrospective analysis.Methods A total of 34 patients with SJS-TEN in our hospital between January 2015 and December 2019 were assessed clinically using the SCORTEN on day one and day three admission.And the association among the SCORTEN values,prognosis and the length of stay in intensive care unit(ICU)and hospital were analyzed.Results SCORTEN value on day one was significantly positively correlated with the length of stay in ICU(r=0.505,P<0.05)and hospital(r=0.530,P<0.05).SCORTEN scores on day three and the length of stay in ICU were significantly positively correlated(r=0.543,P<0.05).Patients with heart rate>120 beats/min and skin exfoliation area>10%total body surface area(TBSA)on day one and day three significantly stayed longer in hospital(P<0.05).Patents with HCO_(3)^(-)<20 mmol/L on day one were observed obvious longer stays in hospital(P<0.001).Conclusions Higher SCORTEN value of SJS-TEN patients on days one and day three might predict worse prognosis and longer length of stay in ICU and hospital.
