摘 要:目的探讨活动性肺结核患者外周血微小RNA-28-3p(miR-28-3p)、miR-93-5p和miR-574-5p表达水平及其临床意义。方法选取2017年11月-2019年11月于河南科技大学第一附属医院行肺结核筛查的300例患者,根据肺结核诊断情况分为活动性肺结核组136例与非活动性肺结核组164例,另选取同期医院体检的102名健康人为对照组。采用逆转录聚合酶链式反应(RT-PCR)检测外周血miR-28-3p、miR-93-5p和miR-574-5p水平,分析活动性肺结核与非活动性肺结核患者外周血miR-28-3p、miR-93-5p和miR-574-5p表达并观察活动性肺结核治疗前后其水平变化,采用受试者工作特征(ROC)曲线分析miR-28-3p、miR-93-5p和miR-574-5p表达水平对区分活动性肺结核疾病预测价值。结果活动性肺结核组吸烟、咳嗽、咯血、肺内空洞、淋巴结肿大占比高于非活性肺结核组(P<0.001),而钙化结节、支气管扩张占比低于非活动性肺结核组(P<0.001);活动性肺结核组干扰素-γ(IFN-γ)、白细胞介素-10(IL-10)、IL-12、红细胞沉降率(ESR)、C-反应蛋白(CRP)与外周血miR-28-3p、miR-93-5p、miR-574-5p相对表达水平高于非活动性肺结核组与对照组(P<0.05),活动性肺结核组治疗后外周血miR-28-3p、miR-93-5p、miR-574-5p相对表达水平均低于治疗前(P<0.05);miR-28-3p、miR-93-5p、miR-574-5p的预测活动性肺结核疾病曲线下面积(AUC)分别为0.906、0.818、0.872(P<0.05),且miR-28-3p的AUC高于miR-93-5p(P=0.002)。结论活动性肺结核患者外周血miR-28-3p、miR-93-5p、miR-574-5p表达水平升高且治疗后呈下降趋势,对区分活动性肺结核存在一定的预测价值。OBJECTIVE To investigate the expression levels of microRNA-28-3 p(miR-28-3 p),microRNA-93-5 p(miR-93-5 p)and microRNA-574-5 p(miR-574-5 p)in peripheral blood of patients with active tuberculosis and their clinical significance.METHODS A total of 300 patients underwent tuberculosis screening in the First Affiliated Hospital of Henan University of Science and Technology from Nov.2017 to Nov.2019 were selected and divided into active tuberculosis group 136 cases and inactive tuberculosis group 164 cases according to the diagnosis of tuberculosis.Another 102 healthy people were selected as the control group.The peripheral blood miR-28-3 p,miR-93-5 p and miR-574-5 p were detected by reverse transcription polymerase chain reaction(RT-PCR).The peripheral blood miR-28-3 p,miR-93-5 p and miR-574-5 p of patients with active tuberculosis and inactive tuberculosis were analyzed,and changes of the three indicators of patients with active tuberculosis before and after treatment were observed.The predictive value of miR-28-3 p,miR-93-5 p and miR-574-5 p for distinguishing active tuberculosis was analyzed by receiver operating characteristic(ROC)curves.RESULTS The proportions of smoking,coughing,hemoptysis,intrapulmonary cavity and lymphadenectasis in active tuberculosis group were significantly higher than those in inactive tuberculosis group(P<0.05),while the proportions of calcified nodules and bronchiectasis were significantly lower than those in inactive tuberculosis group(P<0.05).The contents of interferon-γ(IFN-γ),interleukin-10(IL-10),interleukin-12(IL-12),erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP),as well as relative expression levels of miR-28-3 p,miR-93-5 p and miR-574-5 p in peripheral blood in active tuberculosis group were significantly higher than those in inactive tuberculosis group and control group(P<0.05).After treatment,the relative expression levels of peripheral blood miR-28-3 p,miR-93-5 p and miR-574-5 p in the active tuberculosis group were significanttly lower than those before trea
