摘  要:目的分析肿瘤坏死因子-α(TNF-α)及细胞色素P450酶1A2(CYP1A2)基因多态性与胸腔镜肺叶切除术后肺部感染的关系。方法选择2017年11月-2019年11月于承德市中心医院行胸腔镜肺叶切除术患者156例为研究对象,根据术后是否并发肺部感染将患者分为肺部感染组(n=62)和无肺部感染组(n=94),采用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)分析法检测TNF-α及CYP1A2基因单核苷酸多态性(SNPs)位点多态性,采用Logistic回归分析探讨TNF-α及CYP1A2多态性位点与胸腔镜肺叶切除术后肺部感染的相关性。结果肺部感染组TNF-α基因rs1800629位点GG基因型、G等位基因频率分别为62.90%、77.42%低于无肺部感染组(P<0.05),GA、AA基因型和A等位基因频率分别为29.03%、8.06%和22.58%高于无肺部感染组(P<0.05),rs1800630位点CC、CA、AA基因型和C、A等位基因频率以及rs361525位点GG、GA、AA基因型和G、A等位基因频率与无肺部感染组比较差异无统计学意义;肺部感染组CYP1A2基因rs762551位点CC基因型和C等位基因频率分别为27.42%、51.61%低于无肺部感染组(P<0.05),CA、AA基因型和A等位基因频率分别为48.39%、24.19%和48.39%高于无肺部感染组(P<0.05);TNF-α基因rs1800629位点A等位基因与胸腔镜肺叶切除术后肺部感染风险存在明确相关性(P<0.05),显性模型rs1800629位点多态性差异有统计学意义(P<0.05);CYP1A2基因rs762551位点A等位基因与胸腔镜肺叶切除术后肺部感染风险存在明确相关性(P<0.05)。结论 TNF-α基因rs1800629位点A等位基因以及CYP1A2基因rs762551位点A等位基因与肺癌患者胸腔镜肺叶切除术后肺部感染风险有关。OBJECTIVE To analyze the relationship between gene polymorphisms of tumor necrosis factor-α(TNF-α)and cytochrome P450 enzyme 1 A2(CYP1 A2)and pulmonary infection after thoracoscopic lobectomy.METHODS A total of 156 patients who underwent thoracoscopic lobectomy in the Chengde Central Hospital from Nov.2017 to Nov.2019 were selected as the research subjects and divided into pulmonary infection group(n=62)and non-pulmonary infection group(n=94)according to whether they had pulmonary infection after surgery.Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)analysis method was used to detect the single nucleotide polymorphisms(SNPs)of TNF-αand CYP1 A2 gene,and logistic regression analysis was used to explore the correlation between polymorphism loci of TNF-αand CYP1 A2 and lung infection after thoracoscopic lobectomy.RESULTS The frequencies of GG genotype and frequency of G allele at rs1800629 locus of TNF-αgene in pulmonary infection group were 62.90%and 77.42%,respectively,significantly lower than those in non-pulmonary infection group(P<0.05),while the frequencies of GA and AA genotypes and A allele were 29.03%,8.06%and 22.58%,respectively,significantly higher than those in non-pulmonary infection group(P<0.05),and the frequencies of CC,CA and AA genotypes and C and A alleles at rs1800630 locus and the GG,GA and AA genotypes and G and A alleles frequencies at rs361525 locus in pulmonary infection group were not significantly different from those without pulmonary infection group.The frequencies of CC genotype and C allele at rs762551 locus of CYP1 A2 gene in pulmonary infection group were 27.42%and 51.61%,respectively,significantly lower than those in non-pulmonary infection group(P<0.05),while the frequencies of CA and AA genotypes and A allele were 48.39%,24.19%and 48.39%,respectively,significantly higher than those in non-pulmonary infection group(P<0.05).There was a significant correlation between A allele at rs1800629 locus of TNF-αgene and risk of pulmonary infection after tho
