摘　　要：目的:对腹膜透析病人症状特征进行分类,并探讨不同症状类别病人人口学特征和疾病特征差异。方法:选取上海市3所医院腹膜透析中心或肾内科病房住院的191例腹膜透析病人作为研究对象,采用一般资料调查表、腹膜透析症状负担量表对病人进行调查,采用潜在类别分析将腹膜透析病人按照症状特征进行分类,采用多元Logistic回归探讨不同类别病人人口学特征和疾病特征差异。结果:腹膜透析病人按照症状特征可分为4个潜在类别(P<0.05),根据每个类别的条件概率,分别命名为"症状高发组""低躯体症状、高心理症状组""症状低发组"和"高躯体症状、低心理症状组"。性别、原发病因、Charlson合并症指数及透析龄是影响腹膜透析病人症状特征分类的主要因素。结论:腹膜透析病人有4个潜在类别,腹膜透析病人的心理症状贯穿于透析早中期,且女性风险更高;透析中期病人躯体症状比初期更为严重,合并糖尿病及高合并症指数的腹膜透析病人易处于症状高发水平。Objective:To classify the symptom characteristics of patients with peritoneal dialysis,and to explore differences in demographic characteristics and disease characteristics of patients with different symptom categories.Methods:A total of 191 patients with peritoneal dialysis in peritoneal dialysis center or nephrology department in 3 hospitals in Shanghai,were selected as research subjects.The general information questionnaire and peritoneal dialysis symptom burden scale were used to investigate the patients.Latent class analysis were used to classify patients with peritoneal dialysis according to symptoms.Multivariate Logistic regression was used to explore the differences in demographic characteristics and disease characteristics of patients in different categories.Results:According to symptom characteristics,patients with peritoneal dialysis could be divided into 4 potential categories(P<0.05).According to conditional probability of each category they were named as"symptoms with high incidence group""physical symptoms with low incidence,psychological symptoms with high incidence group""symptoms with low incidence group"and"physical symptoms with high incidence,psychological symptoms with low incidence group".Gender,cause of disease,Charlson comorbidity index and dialysis duration were main influencing factors of classification of symptom characteristics of patients with peritoneal dialysis.Conclusions:Patients with peritoneal dialysis had 4 potential categories.The psychological symptoms of patients with peritoneal dialysis ran through the early and middle stages of dialysis,and females were at higher risk.Physical symptoms of patients in middle stage of dialysis were more serious than those of patients in early stage of dialysis.Peritoneal dialysis patients with diabetes and high comorbidity index tended to be at a level of symptoms with high incidence.
