中药粉末饮片在儿科门诊的应用研究

摘　　要：目的探究中药粉末饮片在儿科门诊的应用效果。方法采用回顾性分析该院2018年1—12月期间儿童内科门诊开出2岁以下婴幼儿处方1087张,统计分析其用药情况。结果通过该院儿内科处方开具调查分析可见:①儿内科开具的大部分处方为呼吸系统疾病,占处方数的96.78%,常用中药粉末饮片为羚羊角粉、蛤蚧粉和制川贝母粉,与病症具有正关联性。②1087张儿科门诊中药粉末饮片处方多由西医师开具,占91.26%,中药粉末饮片联合用药率为99.54%,多与西药联用(联合用药率为47.38%)或与西药、中成药联用(联合用药率为46.09%)。③1087张儿科门诊中药粉末饮片处方中,共有63张不合理处方,占5.80%,其中36张用药不适宜处方,占不合理处方57.14%;超常处方4张,占不合理处方6.35%;不规范处方23张,占不合理处方36.51%。结论儿科门诊内中药粉末饮片多由西医师开具,大多与西药、中成药合用,但不足之处在于西药师在开具处方时,没有充分结合辨证分型开具,因此西药师应加强学习中医药理论有关知识,严格做到辨证施治,合理掌握适应证和禁忌证,加强儿内科应用处方的合理性,提高用药安全度。Objective To explore the application effect of Chinese herbal powder decoction pieces in pediatric clinic.Methods A retrospective analysis of 1087 prescriptions for infants and young children under the age of 2 in the Outpatient Department of Pediatric Internal Medicine of the hospital from January to December 2018 was used to statistically analyze their drug use.Results The investigation and analysis of prescriptions issued by the hospital howed that:1.Most of the prescriptions issued in the pediatric department were respiratory diseases,accounting for 96.78%of the prescriptions.Pediatric medicine powders were commonly used in pediatrics.The decoction pieces were antelope horn powder,gecko powder and Fritillaria powder,which had a positive correlation with the disease.2.1087 pediatric outpatient powdered Chinese medicine powder prescriptions were mostly prescribed by Western physicians,accounting for 91.26%.The combined use rate of powdered Chinese herbal medicine was 99.54%,and most of them were combined with western medicine(the combined use rate was 47.38%)or with western medicine or proprietary Chinese medicine combination(combination rate of 46.09%).3.Among the 1087 pediatric outpatient powdered herbal medicine prescriptions,63 unreasonable prescriptions accounted for 5.80%,of which 36 were inappropriate for prescriptions,accounting for 57.14%of unreasonable prescriptions;4 were supernormal prescriptions,accounting for 6.35%of unreasonable prescriptions;there were 23 irregular prescriptions,accounting for 36.51%of irrational prescriptions.Conclusion In pediatric outpatient clinics,traditional Chinese medicine powdered pieces are mostly prescribed by western physicians,and most of them are combined with western medicine and Chinese patent medicines.However,the disadvantage is that western pharmacists do not fully combine dialectical types to prescribe when they prescribe.Therefore,western pharmacists should strengthen their prescriptions,learn the relevant knowledge of traditional Chinese medicine theory,str
