摘 要:目的分析婴幼儿听力筛查中瞬态诱发耳声发射(TEOAE)联合自动判别听性脑干反应(AABR)的临床应用价值。方法回顾性方便选取2018年1月—2020年1月该院耳鼻喉科婴幼儿1200例(2400耳),均接受TEOAE、AABR、TEOAE联合AABR筛查,分析1200例婴幼儿听力筛查结果、诊断结果。结果1200例婴幼儿2400耳中,TEOAE通过2009耳,TEOAE未通过391耳,分别占总数的83.7%、16.3%;AABR通过2156耳,AABR未通过244耳,分别占总数的89.8%、10.2%;TEOAE联合AABR通过2000耳,TEOAE联合AABR未通过235耳,分别占总数的83.3%、9.8%。TEOAE通过AABR通过2000耳,TEOAE通过AABR未通过9耳,TEOAE未通过AABR通过156耳,TEOAE未通过AABR未通过235耳,分别占总数的83.3%、0.4%、6.5%、9.8%;分泌性中耳炎196耳,感音神经性聋133耳,大前庭水管综合征27耳,小儿听神经病9耳,听力正常2035耳,分别占总数的8.2%、5.5%、1.1%、0.4%、84.8%。结论婴幼儿听力筛查中TEOAE联合AABR的临床应用价值较单独筛查高,更能为早期发现、诊断、干预听力障碍婴幼儿提供有效依据,值得在临床推广应用。Objective To analyze the clinical application value of TEOAE combined with AABR in infant hearing screening.Methods A retrospective selection of 1200 infants(2400 ears)in the Department of Otolaryngology in the hospital from January 2018 to January 2020 were conveniently selected for TEOAE,AABR,TEOAE combined with AABR screening,and 1200 infants’hearing screening results and diagnosis were analyzed.Results Among the 2400 ears of 1200 infants,TEOAE passed 2009 ears and TEOAE failed 391 ears,accounting for 83.7%and 16.3%of the total;AABR passed 2156 ears and AABR failed 244 ears,accounting for 89.8%and 10.2%of the total;TEOAE combined with AABR passed 2000 ears,TEOAE combined with AABR failed 235 ears,accounting for 83.3%and 9.8%of the total respectively.TEOAE passed AABR for 2000 ears,TEOAE passed AABR but failed 9 ears,TEOAE failed AABR passed 156 ears,TEOAE failed AABR failed 235 ears,accounting for 83.3%,0.4%,6.5%,9.8%of the total respectively;secretory 196 ears with otitis media,133 ears with sensorineural hearing loss,27 ears with large vestibular aqueduct syndrome,9 ears with pediatric auditory neuropathy,and 2035 ears with normal hearing,accounting for 8.2%,5.5%,1.1%,0.4%,and 84.8%of the total respectively.Conclusion The clinical application value of TEOAE combined with AABR in infant hearing screening is higher than that of screening alone.It can provide an effective basis for early detection,diagnosis and intervention of hearing-impaired infants and young children,and it is worthy of clinical application.
