改良型穿刺法用于经阴道注水腹腔镜术的效果评价

摘　　要：目的评价改良型穿刺法应用于经阴道注水腹腔镜术(THL)的有效性及安全性。方法方便选取2018年1月—2019年12月两年在该院行改良穿刺法后THL治疗的80例患者的结局,记录穿刺成功率、穿刺并发症转行腹腔镜(LAP)率、常见并发症的发生率,从而评价该方法的有效性及安全性。结果该研究共纳入需行THL的患者80例,改良穿刺法成功率为100.00%,其中1例因穿刺中出现血性液体转开腹,LAP率为1.25%,穿刺相关并发症(直肠穿孔、穿刺部位出血、穿刺点周围组织血肿、子宫后壁或肌层损伤)发生率为2.50%。结论改良型穿刺法,可以提高经阴道注水腹腔镜术穿刺的有效性和安全性,值得推广使用。Objective To evaluate the effectiveness and safety of the modified puncture method used in transvaginal laparoscopic surgery(THL).Methods Statistics of the outcomes of 80 patients treated by THL in the hospital from January 2018 to December 2019 during the two years of improved puncture were conveniently selected,and the success rate of puncture and the rate of puncture complications converted to laparoscopy(LAP),the incidence of common complications were recorded,so as to evaluate the effectiveness and safety of the method.Results A total of 80 patients who required THL included in this study.The success rate of the modified puncture method was 100.00%.Among them,1 case was converted to open abdomen due to bloody fluid during puncture.The LAP rate was 1.25%.The puncture-related complications(The incidence of rectal perforation,bleeding at the puncture site,hematoma around the puncture site,injury to the posterior wall of the uterus or muscle layer)was 2.50%.Conclusion The improved puncture method can improve the effectiveness and safety of transvaginal water injection laparoscopic puncture,and it is worthy of popularization.
