摘　　要：目的探讨失效模式与效应分析(FMEA)在骨科手术患者围手术期转运交接过程中应用效果。方法基于FMEA模式,评估RPN值≧240分的失效模式,分析各环节失效的原因并进行改进,比较实施FMEA前后的RPN值、手术周转和投诉情况。结果实施FMEA后,骨科手术患者转运交接风险RPN值(x±s)均较实施前降低,实验组每日手术台数和每日每间手术台数明显高于对照组,其差异有统计学意义(t=22.02,t=15.37,P<0.01),实验组总投诉事件明显低于对照组,其差异有统计学意义(χ^(2)=8.962,P<0.01)。结论基于FMEA模式的骨科手术患者全程转运交接,不仅减少了护理差错和投诉事件的发生,还提高了手术间的使用率,保障了患者的安全,提升了护理质量。Objective To explore the application effect of failure mode and effect analysis(FMEA)mode in perioperative transport and handover of orthopaedic surgery patients.Methods Based on FMEA mode,the failure mode of RPN values great than or equal to 240 points were evaluated,the causes of failure of each link were analyzed and improved,and RPN values,operation turnover and complaints were compared before and after implementation FMEA.Results After implementation of FMEA,the RPN values of transfer and handover risk(x±s)in patients undergoing orthopaedic surgery were all lower than those before the implementation of FMEA.The daily number of surgicals and the daily number of each surgical every operating room in the experimental group were significantly higher than those in the control group,and the difference was statistically significant(T=22.02,T=15.37,P<0.01).The total complaint events in the experimental group were significantly lower than those in the control group,and the difference was statistically significant(χ^(2)=8.962,P<0.01).Conclusion Based on FMEA mode,the whole-process transfer and handover of orthopaedic surgery patients can not only reduce the occurrence of nursing errors and complaints,but also improve the utilization rate between surgeries,ensure the safety of patients and improve the quality of nursing.
