无痛消化内镜下高频电刀治疗胃肠息肉的效果研究

摘　　要：目的分析胃肠息肉在无痛消化内镜下进行高频电刀治疗的效果。方法70例胃肠息肉患者,随机分为研究组和对照组,各35例。研究组在无痛消化内镜下进行高频电刀治疗,对照组在普通消化内镜下进行高频电刀治疗。对比两组手术前后的胃肠道激素[胃泌素(GAS)、胃动素(MOT)、血管活性肠肽(VIP)]水平、一次性息肉清除率、创面愈合率、复发率、手术时间、术后肠鸣音恢复时间、排便时间。结果术后,两组MOT、GAS水平均较术前降低,VIP水平均较术前升高,但研究组MOT(243.15±22.76)pg/ml、GAS(131.65±16.71)pg/ml高于对照组的(228.68±20.15)、(116.21±15.81)pg/ml,VIP(41.21±6.51)pg/ml低于对照组的(46.08±6.32)pg/ml,差异均有统计学意义(P<0.05)。两组患者的一次性息肉清除率、创面愈合率、复发率对比,差异无统计学意义(P>0.05)。研究组手术时间、术后肠鸣音恢复时间、排便时间分别为(26.85±5.39)min、(9.26±3.88)h、(41.26±12.08)h,均短于对照组的(30.59±5.41)min、(12.32±4.21)h、(50.36±12.21)h,差异有统计学意义(P<0.05)。结论相比于传统内镜下高频电刀治疗,在无痛消化内镜下进行高频电刀治疗胃肠息肉效果显著,对患者的胃肠道激素水平影响更小,胃肠功能恢复更快。Objective To analyze the effect of high-frequency electric knife on gastrointestinal polyps under painless digestive endoscopy.Methods A total of 70 patients with gastrointestinal polyps were randomly divided into research group and control group,with 35 cases in each group.The research group was treated with high-frequency electric knife under painless digestive endoscopy,and the control group was treated with highfrequency electric knife under general digestive endoscopy.The gastrointestinal hormone[gastrin(GAS),motilin(MOT),vasoactive intestinal peptide(VIP)]before and after operation,one-time polyp clearance rate,wound healing rate,recurrence rate,operation time,postoperative bowel sound recovery time,and defecation time were compared between the two groups.Results After operation,the MOT and GAS of the two groups were lower than those before operation,and the VIP was higher than that before operation;the MOT(243.15±22.76)pg/ml and GAS(131.65±16.71)pg/ml of the research group were higher than(228.68±20.15)and(116.21±15.81)pg/ml of the control group,and the VIP(41.21±6.51)pg/ml was lower than(46.08±6.32)pg/ml of the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in one-time polyp clearance rate,wound healing rate and recurrence rate between the two groups(P>0.05).The operation,postoperative bowel sound recovery time and defecation time of the research group were(26.85±5.39)min,(9.26±3.88)h and(41.26±12.08)h,which were shorter than(30.59±5.41)min,(12.32±4.21)h and(50.36±12.21)h of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with high-frequency electric knife under traditional endoscopy,the high-frequency electric knife under painless digestive endoscopy has a significant effect for patients with gastrointestinal polyps and a smaller effect on the gastrointestinal hormone levels of the patients,and the intestinal function of patients recovers faster.
