摘  要:目的探讨电针联合经颅磁刺激对血管性痴呆(VaD)大鼠学习记忆能力的影响及其机制。方法雄性Sprague-Dawley(SD)大鼠48只,8周龄,体质量240~280 g,采用随机数字表法分为4组:假手术组(S组)、模型组(M组)、电针组(E组)和电针+磁刺激组(EM组),每组12只。采用四血管阻断法构建大鼠VaD模型,E组大鼠取"百会穴"和"大椎穴",电针30 min/次,1次/d,连续14 d;EM组大鼠在电针干预后,接受经颅磁刺激,100个磁脉冲/次,1次/d,连续14 d;S组和M组大鼠以相同的方式和时间固定于柔软性固定器中而不给予其他干预。采用Morris水迷宫实验测试大鼠学习记忆能力,脱氧核苷酸末端转移酶介导的2′-脱氧尿苷5′-三磷酸盐缺口末端标记(TUNEL)法检测大鼠海马CA1区细胞凋亡情况,Western blot法检测海马组织中磷脂酰肌醇3激酶(PI3K)、蛋白激酶B(Akt)、磷酸化Akt(p-Akt)、哺乳动物雷帕霉素靶蛋白(mTOR)和磷酸化mTOR(p-mTOR)蛋白相对含量,计算p-Akt/Akt和p-mTOR/mTOR的比值。结果与S组比较,M组、E组和EM组逃避潜伏期[(48.9±2.2)、(40.8±2.2)、(36.7±2.4)s比(33.6±1.6)s]和第1次跨越平台时间[(54.8±1.8)、(37.8±2.2)、(30.2±1.8)s比(24.5±1.8)s]均明显延长(均P<0.05),而2 min内跨越原平台次数[(3.2±0.8)、(5.6±0.8)、(7.6±1.0)次比(10.2±0.8)次]明显减少(均P<0.05),细胞凋亡指数明显升高(均P<0.05),PI3K蛋白相对含量明显升高(均P<0.05),而p-Akt/Akt值和p-mTOR/mTOR值明显降低(均P<0.05);与M组比较,E组和EM组逃避潜伏期和第1次跨越平台时间均明显缩短(均P<0.05),而2 min内跨越原平台次数明显增加(均P<0.05),细胞凋亡指数明显降低(均P<0.05),PI3K蛋白相对含量明显降低(均P<0.05),而p-Akt/Akt和p-mTOR/mTOR比值明显升高(均P<0.05);与E组比较,EM组逃避潜伏期和第1次跨越平台时间均明显缩短(均P<0.05),而2 min内跨越原平台次数明显增加(P<0.05),细胞凋亡指数明显降低(P<0.05),PI3K蛋白相对含量明显降�Objective To investigate the effects of electroacupuncture combined with transcranial magnetic stimulation on learning-memory ability in rats with vascular dementia(VaD),and possible mechanisms.Methods A total of 48 healthy male Sprague-Dawley(SD)rats,240-280 g,eight weeks,were randomly divided into four groups:sham-operated group(group S),model group(group M),electroacupuncture group(group E)and electroacupuncture+magnetic stimulation group(group EM),with 12 rates in each group.The VaD model was constructed by four-vessel occlusion method.DU20(BaiHui)and DU14(DaZhui)in the electroacupuncture group were acupunctured 30 min/time and 1 time/d for 14 consecutive days in group E.The rats in the group EM received transcranial magnetic stimulation after electroacupuncture intervention,100 magnetic pulses/time and 1 time/d for 14 consecutive days.The rats in the S group and the M group were fixed in the same manner and at the same time in the softness holder without giving other interventions.The learning-memory ability in rats was detected by Morris water maze test.The neuronal apoptosis in hippocampal CA1 region of rats was detected by terminal deoxynucleotidyl transferase-mediated 2′-deoxyuridine 5′-triphosphate nick-end labeling(TUNEL)assay.Western blot method was used to detect the relative content of phosphatidylinositol 3-kinase(PI3 K),protein kinase B(Akt),phospho-Akt(p-Akt),mammalian target of rapamycin(mTOR)and phospho-mTOR(p-mTOR)proteins in hippocampus,and the ratio of p-Akt/Akt and the ratio of p-mTOR/mTOR were calculated.Results Comparing the group M,group E and group EM with the group S,the escape latency time([48.9±2.2]s,[40.8±2.2]s and[36.7±2.4]s vs.[33.6±1.6]s)and the first crossing time([54.8±1.8]s,[37.8±2.2]s and[30.2±1.8]s vs.[24.5±1.8]s)were obviously extended(all P<0.05),while the numbers of crossing the original platform within 2 minutes([3.2±0.8],[5.6±0.8]and[7.6±1.0]vs.[10.2±0.8])were obviously decreased(all P<0.05);the neuronal apoptosis indexes in the hippocampal CA1 regions o
