林毅“六郁治乳”理论在乳腺病诊治中的应用

摘　　要：林毅教授在系统继承历代医家郁证学说精华的基础上,结合长期临床实践,拓展了郁证学说在乳腺病中的运用,提出了“六郁治乳”理论。认为中医治疗乳病可从气郁、血郁、火郁、痰郁、食郁、湿郁“六郁辨之”,乳房疾病的产生主要与肝、脾(胃)、肾及冲任等经络相关;乳腺疾病可从“六郁治之”,确立了“明辨六郁,详察虚实,标本兼顾,平衡调治”的总体治则,临床诊疗可采用祛除诸郁之疏肝、祛瘀、清热、化痰、利湿、消食、健脾、补肾等“八法”,以平衡调治为宗。“六郁治乳”理论为构建中医乳房病学系统的理法方药辨治体系提供了参考。By systematically inheriting the essence of the depression syndrome theory of the past generations,and by combining years of clinical experience, Professor LIN Yi expanded the application of depression syndrome theory in the treatment of breast diseases, and propounded “six kinds of stagnancy theory for breast”. In the theory of “six kinds of stagnancy theory for breast”, it is suggested that the breast diseases in the field of traditional Chinese medicine(TCM)can be differentiated into qi stagnation,blood stagnation,fire stagnation,phlegm stagnation,indigestion-induced qi stagnation and dampness stagnation,and the breast diseases mainly involve the liver meridian, spleen(stomach)meridian, kidney meridian, thoroughfare vessel and conception vessel. The therapeutic principles for the breast diseases by “six kinds of stagnancy theory for breast” are to distinguish six kinds of stagnancy exactly,differentiate the excess and deficiency actually,treat the primary and secondary symptoms simultaneously,and regulate equivalently. Eight methods for relieving stagnancy consist of soothing liver, removing blood stasis, clearing heat, resolving phlegm, promoting digestion, invigorating spleen,and tonifying kidney,and the treatment aims at regulating equivalently. “Six kinds of stagnancy theory for breast” will supply references for constructing the syndrome differentiation and treatment system of theory,therapeutic principles, prescription, and medication for the treatment of traditional Chinese medicine breast diseases.
