黄枫治疗肩周炎用药规律分析

摘　　要：基于中医传承辅助平台软件(V2.5),采用频次分析、关联规则分析、聚类分析等方法分析黄枫教授治疗肩周炎患者的有效病例的门诊处方,探讨其治疗肩周炎的用药规律和诊治思路。结果共筛选出处方160首,涉及中药72味。使用频次居前10位的中药分别为甘草、延胡索、玉竹、桑枝、生地黄、两面针、石斛、葛根、薏苡仁、丹参。23味常用药物根据其功效可分为健脾祛湿药、健脾益气药、活血止痛药、舒筋通络药、养阴柔筋药、补肾药。基于聚类分析演化得到7个新方组合。分析结果提示黄枫教授治疗肩周炎用药以活血行气止痛、养阴养血柔筋为主,辅以健脾益气祛湿与补肾,体现其治疗肩关节周围炎以气血筋脉为切入点,从脾肾两脏论治,兼顾先后天之本的临证经验以及结合岭南地域气候特点的辨证思想。By using the methods of frequency analysis, association analysis, and cluster analysis in the Traditional Chinese Medicine Inheritance Support System(V2.5),we analyzed the prescriptions for the effective cases of the out-patients with frozen shoulder treated by Professor HUANG Feng,so as to explore the medication rule and therapeutic thoughts of Professor HUANG Feng for the treatment of frozen shoulder. The results showed that a total of 160 prescriptions and 72 herbs were screened out. The leading 10 herbs with high medication frequency were Radix Glycyrrhizae, Rhizoma Corydalis, Rhizoma Polygonati Odorati, Ramulus Mori, Radix Rehmanniae, Radix Zanthaxyli, Herba Dendrobii, Radix Puerariae, Semen Coicis, and Radix Salviae Miltiorrhizae. The 23 commonly-used herbs can be classified into the categories of herbs for invigorating spleen and removing dampness,herbs for invigorating spleen and replenishing qi,herbs for activating blood and relieving pain,herbs for relaxing tendons and unblocking collaterals,herbs for nourishing yin and soothing tendons,and herbs for tonifying kidney according to their therapeutic actions. Seven new candidate recipes were derived from the results of cluster analysis. The results indicated that the medication by Professor HUANG Feng for the treatment of frozen shoulder is characterized by the utilization of herbs for activating blood,regulating qi and relieving pain,and herbs for nourishing yin and blood and soothing tendons,which were assisted by the herbs for invigorating spleen and replenishing qi and herbs for tonifying kidney. And the results also indicated that the thoughts of Professor HUANG Feng for the treatment of frozen shoulder are based on the theory of qi,blood and tendons,and stress the importance of simultaneous treatment of spleen and kidney, tonifying both congenital and acquired foundation of life,and taking the climatic features of Lingnan region into account.
