摘 要:目前对存在高出血风险的血液透析患者,枸橼酸局部抗凝(RCA)已成为首选的临床抗凝方案。RCA可以在透析装置局部发挥抗凝作用而避免造成患者全身的凝血抑制状态,与低分子肝素或无抗凝的血液透析比较,其临床疗效更加显著。近年来,RCA技术已逐渐简化,应用也愈加成熟,加之涌现了很多专用于RCA的商用透析液、透析机和与之相匹配的集成输液系统及管理软件,都使得RCA的临床应用更加便捷和安全。本文对RCA技术在血液透析中的应用机制及安全性进行综述。At present,regional citrate anticoagulation(RCA)has become the first choice for hemodialysis patients with high risk of bleeding.RCA can play a local anticoagulant role in the dialysis device to avoid causing systemic coagulation inhibition.Compared with low molecular weight heparin or non anticoagulant hemodialysis,RCA has more significant clinical effect.In recent years,RCA technology has been gradually simplified and its application has become more mature.In addition,a lot of commercial dialysate and dialyzer specially used for RCA and the corresponding integrated infusion system and management software have emerged,which makes the clinical application of RCA more convenient and safe.This article aims to review the application mechanism and safety of citrate local anticoagulant technology in hemodialysis.
