摘  要:目的研究特发性肺纤维化(idiopathic pulmonary fibrosis,IPF)患者血清中缺氧诱导因子-1α(hypoxia inducible factor,HIF-1α)、特异性蛋白酶1(SENP1)表达水平的变化,探讨其在特发性肺纤维化中的临床意义。方法选取IPF患者30例为观察组,同时收集30例健康体检者作为对照组。采用酶联免疫吸附法(ELISA)检测两组患者血清中HIF-1α、SENP1蛋白表达水平。线性分析HIF-1α与SENP1表达水平的相关性。回归分析HIF-1α与SENP1表达量与肺功能指标[用力肺活量占预计值的百分比(FVC% pred)及一氧化碳弥散量占预计值的百分比(DLco% pred)]的相关性。稳定期和急性加重期IPF患者的血清HIF-1α、SENP1水平进行对比分析。结果观察组患者肺功能指标(FVC% pred、DLco% pred)显著低于对照组(P<0.01),而血清HIF-1α、SENP1表达水平则显著高于对照组(P<0.01)。观察组中血清HIF-1α水平随SENP1水平升高而升高,两者呈显著正相关(r=0.970,P=0.001)。HIF-1α、SENP1表达水平与FVC% pred(HIF-1α:r=-0.878,P=0.001;SENP1:r=-0.888,P=0.001)、DLco%pred(HIF-1α:r=-0.940,P=0.001;SENP1:r=-0.953,P=0.001)分别呈显著负相关。与稳定期IPF患者相比,急性加重期IPF患者血清HIF-1α、SENP1水平进一步升高(P<0.01)。结论IPF患者血清HIF-1α和SENP1表达水平的高低与肺功能参数、病情严重程度之间有一定相关性,可用作早期诊断、药靶研发的生物学指标。Objective To study the expression of HIF-1αand SENP1 in the serum of patients with idiopathic pulmonary fibrosis(IPF)and its clinical significance.Methods Thirty patients with IPF were selected as the IPF group,and 30 healthy persons as the control group.The levels of HIF-1αand SENP1 in serum of the two groups were assessed by enzyme-linked immunosorbent assay(ELISA).Linear analysis of the correlation between HIF-1α and SENP1 expression was conducted.Regression analysis of the correlations of the expression of HIF-1α and SENP1 with pulmonary function indexes(FVC% PRED and DLCO% PRED)was also conducted.Comparative analysis of HIF-1α and SENP1 levels in patients with IPF in stable and acute exacerbations was performed.Results Between the control group and the observation group(IPF),there was no significant difference in gender,age or smoking history(P>0.05).The lung function indexes(FVC% PRED and DLCO% PRED)in the observation group were significantly lower than those in the control group(P<0.01).The levels of serum HIF-1α and SENP1 in the observation group were significantly higher than those in the control group(HIF-1α:t=32.390,P<0.01;SENP1:t=58.830,P<0.01).In the observation group,the level of serum HIF-1α was increased with the increase of SENP1 level,showing a significant positive correlation(r=0.970,P=0.001).HIF-1αand SENP1 expression levels were significantly negatively correlated with FVC% pred(HIF-1α:r=-0.878,P=0.001;SENP1:r=-0.888,P=0.001)and DLco% pred(HIF-1α:r=-0.940,P=0.001;SENP1:r=-0.953,P=0.001).Compared with patients with stable IPF,serum levels of HIF-1α and SENP1 in patients in acute exacerbation phase were significantly increased(all P<0.01).Conclusion The levels of serum HIF-1α and SENP1 expression in patients with IPF are correlated with pulmonary function parameters and disease severity,which can be used as biological indicators for early diagnosis and drug target development.
