摘 要:目的观察电针心经对急性心肌缺血(acute myocardial ischemia,AMI)大鼠内侧隔核(medial septal,MS)区5-羟色胺(5-HT)与c-fos蛋白的影响,探索电针心经抗心肌缺血作用中MS发挥的作用及其机制。方法选用雄性健康SD大鼠,将其随机分成伪手术组(sham operation group)、模型组(model group)与针刺组(acupuncture group),选取每组6只。心肌缺血模型的复制选择手术法结扎冠状动脉左前降支。选取手少阴心经"神门-通里"段为针刺组,电针时间30 min,连续电针3 d,刺激强度1 mA,疏密波形,调节频率2 Hz。另两组不作电针处理。用Powerlab16导生理记录仪记录心电图(ECG);使用酶联免疫吸附法(ELISA)检测血清乳酸脱氢酶(LDH)的水平与内侧隔核(MS)区5-HT的含量;内侧隔核区c-fos蛋白含量用免疫印迹蛋白(Western blot)技术检测;大鼠心肌组织形态变化运用HE染色法观察。结果造模后与造模前比较:(1)大鼠心电图ST段抬高(≥0.1 mV);(2)血清LDH水平显著升高(P<0.01);(3)多数心肌组织结构明显破坏,排列紊乱的心肌纤维有断裂形态,细胞核分布不集中、状态混乱;(4)MS区5-HT含量明显下降(P<0.05),c-fos蛋白含量升高(P<0.01)。针刺组与模型组比较:(1)血清LDH水平下降明显(P<0.01);(2)心肌病理形态有所改善,表现为心肌纤维排列相对规整,细胞炎性浸润减少;(3)MS区5-HT含量明显上升(P<0.05),c-fos蛋白含量显著下降(P<0.01)。结论内侧隔核参与电针心经改善急性心肌缺血效应,其作用机制可能与针刺通过提高中枢MS区的5-HT含量,降低c-fos蛋白表达有关。Objective To observe the effect of electroacupuncture on the serotonin(5-HT)and c-fos protein in the medial septal nucleus(MS)of rats with acute myocardial ischemia(AMI),and to explore the effect of MS in the anti-myocardial ischemia of acupuncture mechanism. Methods Male SpragueDawley rats were randomly divided into sham operation group,model group and acupuncture group,with 6 rats in each group. The left anterior descending coronary artery was ligated. The"Shenmen(HT7)-Tongli(HT5)"segment of the hand Shaoyin heart meridian was selected as the acupuncture group. The electro acupuncture time was 30 min,and the continuous electro acupuncture was performed for 3 days. The stimulation intensity was 1 mA,the waveform was sparse and dense, and the frequency was adjusted to 2 Hz. The other two groups were not treated with electroacupuncture. Recording an electrocardiogram(ECG)with a Powerlab 16 guided physiological recorder;Serum lactate dehydrogenase(LDH)levels and the content of 5-HT in the medial septal(MS)region were detected by enzyme-linked immunosorbent assay(ELISA). The content of c-fos protein in the medial compartment was detected by Western blot. The morphology of myocardial tissue was observed by HE staining. Results After modeling compared with before modeling:(1)The ST segment elevation in electrocardiogram of rats was ≥ 0.1 mV;(2)the serum LDH level was significantly increased(P<0.01);(3)the structure of most myocardial tissues was obviously destroyed,the disordered arrangement of myocardial fibers had broken shape,and the distribution of nuclei was not concentrated;(4)the 5-HT content in the MS area decreased significantly(P<0.05),c-fos protein content increased significantly(P<0.01). Acupuncture group compared with the model group:(1)the serum LDH level decreased significantly(P<0.01),(2)the pathological morphology of myocardium was improved,which showed relatively regular arrangement of myocardial fibers and reduced inflammatory infiltration of cells;(3)the 5-HT content in the MS area increased sign
