针刺联合息痛散对急性痛风性关节炎患者炎症因子、氧化应激表达的影响

摘　　要：目的探究针刺联合息痛散对急性痛风性关节炎患者临床疗效以及其对炎症因子和氧化应激表达的影响。方法依据纳入、排除标准,筛选我院2019年7月—2020年4月期间收治的急性痛风性关节炎患者,最终入组90例,按随机法进行分组,其中对照组45例予常规西医治疗,治疗组在其基础上加用针刺联合息痛散治疗,疗程均为7 d。观察两组患者中医证候积分改变情况,治疗前后患者关节疼痛、肿胀评分,理化实验、外周血炎症因子及氧化应激表达指标改善情况,并比较两组临床疗效。结果治疗组关节疼痛、关节肿胀、发热口渴、小便黄赤等中医证候积分降低较对照组更显著(P<0.05);治疗组关节疼痛视觉模拟评分(VAS)、关节肿胀评分下降幅度较对照组更明显(P<0.05);治疗组超敏C反应蛋白(hs-CRP)、尿酸(UA)、血沉(ESR)、白细胞介素1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)、3-硝基酪氨酸(3-NT)和8-羟化脱氧鸟苷(8-OHdG)等理化实验指标均较对照组下降趋势更显著(P<0.05);治疗组临床总有效率高于对照组(95.56%vs 80.00%)(P<0.05)。结论针刺联合息痛散具有清热利湿、宣痹通络之效,可以缓解急性痛风性关节炎患者关节疼痛,抑制炎症反应,调节氧化应激表达,治疗急性痛风性关节炎效果较好,值得临床推广。Objective The purpose of this study was to explore the effect of acupuncture combined with Xitong Powder(息痛散)on expression of inflammatory factors and oxidative stress with acute gouty arthritis. Methods According to inclusion criteria and exclusion criteria,collected the information of acute gouty arthritis patients who were diagnosed and treated in our hospital from July 2019 to April 2020. 90 qualified patients were divided into two groups according to random method[control group(group A)vs. treatment group(group B)]. 45 patients in the group A were treated with conventional Western medicine treatment(scheme A). 45 patients in the group B were treated with acupuncture combined with Xitong Powder plus scheme A. 7 days for a course. TCM syndrome scores,the scores of joint pain and swelling,the results of physical and chemical experiments,the improvement of inflammatory factors and oxidative stress expression indexes in peripheral blood before and after treatment,and the clinical effects were observed. Results After treatment,the results of the group B compared with the group A were as follows:Chinese medicine symptoms such as joint pain,joint swelling,fever and thirst,and short voidings of reddish urine were improved in both groups,and the TCM syndrome scores decreased(P<0.05),especially in the group B(P<0.05). The VAS score of joint pain and score of joint swelling in both groups showed a downward trend(P<0.05),especially in group B(P<0.05). The levels of hs-CRP,UA,ESR,IL-1β,TNF-α,IL-6,3-NT and 8-OHdG in both groups were lower than those before treatment(P<0.05). Compared between groups,the decline trend of each index in group B was more significant(P<0.05). The clinical efficacy of group B(95.56%)was better than that of group A(80.00%).
