摘 要:目的观察"前后配穴"电针对前列腺增生(benign prostatic hyperplasia,BPH)大鼠前列腺的湿重、指数、组织病理学和血清中睾酮(T)、Ⅱ型5α还原酶(SRD5α2)、双氢睾酮(DHT)、前列腺酸性磷酸酶(PAP)含量的影响,探讨"前后配穴"电针对BPH大鼠的治疗作用及机制。方法 SD雄性大鼠随机分为空白组、模型组、非那雄胺(1 mg/kg)组和电针组,每组8只,采用连续4周皮下注射丙酸睾酮(5 mg/kg)法,制造BPH大鼠模型。造模同时给予大鼠相应治疗,非那雄胺组给予非那雄胺1 mg/kg灌胃;电针组取"中极、关元"和"肾俞、会阳"两组腧穴隔天交替针刺,20 min/次,1次/d,共治疗4周。治疗结束后,称量大鼠体质量与前列腺湿重,计算前列腺指数,采用HE染色法观察前列腺组织形态学变化,酶联免疫吸附法测定血清中T、SRD5α2、DHT及PAP含量。结果与空白组比较,模型组大鼠前列腺湿重与前列腺指数显著升高(P<0.01),组织病理学显示腺上皮显著增厚(P<0.01),腺腔面积显著扩张(P<0.01),血清各项指标亦明显升高(P<0.05);与模型组相比,电针组与非那雄胺组大鼠前列腺湿重与前列腺指数均显著降低(P<0.01),腺上皮显著变薄(P<0.01),腺腔面积明显减小(P<0.05),血清指标除T有下降趋势但差异无统计学意义(P>0.05)外,其余各项指标均显著降低(P<0.01或P<0.05)。与非那雄胺组相比,电针组大鼠各项指标差异均无统计学意义(P>0.05)。结论 "前后配穴"电针可有效防治大鼠BPH,其机制可能与降低BPH大鼠血清中SRD5α2含量,进而减少T向DHT的转化,减轻DHT对前列腺组织的刺激有关。Objective To observe the distribution"anterior-posterior point association" electroacupuncture for benign prostatic hyperplasia(BPH)rat prostate wet weight,index,histopathological and the influence of the serum content of testosterone(T),Ⅱ type 5α reductase(SRD5α2),dihydrotestosterone(DHT),prostatic acid phosphatase(PAP). To explore the"anterior-posterior point association"electroacupuncture for BPH the therapeutic effect of rats and its mechanism. Methods SD male rats were randomly divided into blank group,model group,finasteride(1 mg/kg)group and electroacupuncture group,with 8 rats in each group. The rat model of BPH was prepared by subcutaneous injection of 5 mg/kg propionate for 4 week. At the same time,the rats were given corresponding treatment,and the rats in the finasteride group were given 1 mg/kg of finasteride by gavage. In the electroacupuncture group,"Zhongji(RN3),Guanyuan(RN4)"and"Shenshu(BL23),Huiyang(BL35)"acupoints were applied alternately every other day,20 min/time,once/day,for a total of 4 week. After the treatment,the weight of the rats and the wet weight of the prostate were weighed,the prostate index was calculated,the morphological changes of the prostate were observed by HE staining,and the contents of T,SRD5α2,DHT and PAP in the serum were determined by Elisa. Results Compared with the blank group,the wet weight and prostate index of the model group were significantly increased(P<0.01),histopathology showed significant thickening of glandular epithelium(P<0.01),the gland cavity area was significantly expanded(P<0.01),and the serum indexes were significantly increased(P<0.05). Compared with the model group,the wet weight and prostate index of the rats in the finasteride group and the electroacupuncture group were significantly reduced(P<0.01),the glandular epithelium was significantly thinned(P<0.01),and the area of the glandular cavity was significantly reduced(P<0.05).
