穴位贴敷联合补肾活血法对ACA阳性复发性流产患者内分泌调节、细胞免疫学及β-HCG、ACA、CA125水平的影响

摘　　要：目的研究穴位贴敷联合补肾活血法治疗抗心磷脂抗体(ACA)阳性复发性流产患者疗效情况,分析其对患者内分泌调节、细胞免疫学及β-绒毛膜促性腺激素(β-HCG)、ACA、糖类抗原-125(CA125)水平的影响。方法选取2017年6月—2019年1月在该院接受治疗的95例ACA阳性复发性流产患者作为研究对象,根据患者治疗方式的差异,将采用小剂量阿司匹林治疗的43例患者纳入对照组,在对照组基础上采用穴位贴敷联合补肾活血法治疗的52例患者纳入研究组。两组患者均确定为宫内妊娠后开始治疗,在孕12周时结束治疗。对两组患者疗效情况、血清激素水平、生化指标、T淋巴细胞亚群水平及妊娠结局进行分析比较。结果治疗后,研究组孕酮(P)、雌二醇(E2)、β-HCG、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)水平均明显高于对照组(P<0.05),ACA、CA125、白细胞介素-17(IL-17)水平均明显低于对照组(P<0.05);研究组CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)均明显高于对照组(P<0.05),CD_(8)^(+)明显低于对照组(P<0.05);研究组治疗总有效率为90.38%,明显高于对照组72.09%(P<0.05),研究组妊娠成功率为90.38%,明显高于对照组72.09%(P<0.05)。结论穴位贴敷联合补肾活血法可以调节ACA阳性复发性流产患者的内分泌功能,纠正机体免疫失调,下调炎症因子水平,从而提高治疗效果,改善患者妊娠结局。Objective To observe the therapeutic effect of acupoint application combined with kidneytonifying and blood-activating method in the treatment of recurrent abortion with positive anticardiolipin antibody(ACA),and analyze its influence on endocrine regulation,cellular immunology and levels of β-hCG,ACA and CA125. Methods 90 patients with ACA positive recurrent abortion who were treated in our hospital from June 2017 to January 2019 were selected as the research objects. According to the difference of treatment methods,45 patients treated with low-dose aspirin were included in the control group. On the basis of the control group,45 patients were treated with acupoint application combined with kidneytonifying and blood-activating method. Two groups of patients were confirmed to be intrauterine pregnancy after treatment,at the end of 12 weeks of pregnancy. The curative effect,serum hormone level,biochemical indexes,T lymphocyte subsets level and pregnancy outcome of the two groups were analyzed and compared. Results After treatment,the levels of progesterone(P),estradiol(E2),β-hCG,interleukin-4(IL-4),interleukin-10(IL-10) in the study group were significantly higher than those in the control group(P<0.05),the levels of ACA,CA125 and IL-17 in the study group were significantly lower than those in the control group(P<0.05);the levels of CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+) in the study group were significantly higher than those in the control group(P<0.05),and CD_(8)^(+) in the study group was significantly higher than that in the control group(P<0.05). The total effective rate of the study group was 90.38%,significantly higher than that of the control group 72.09%(P<0.05),and the pregnancy success rate of the study group was 90.38%,significantly higher than that of the control group 72.09%(P<0.05). Conclusion Acupoint application combined with kidney-tonifying and blood-activating method can regulate the endocrine function of patients with ACA positive recurrent abortion,correct the immune imbalance,and lower the level
