茵陈术附汤加减治疗乙型肝炎相关性慢加急性肝衰竭阴阳黄证的疗效观察

摘　　要：目的:观察茵陈术附汤加减治疗乙型肝炎相关性慢加急性肝衰竭阴阳黄证的临床疗效。方法:将80例乙型肝炎相关性慢加急性肝衰竭患者按随机数字表分为对照组和治疗组各40例,对照组患者予西医综合治疗,治疗组患者在西医综合治疗基础上联合应用茵陈术附汤加减治疗,疗程为4周。观察比较治疗前后两组患者中医证候积分、肝功能(TBIL、ALT、ALB)、凝血酶原活动度(PTA),并评价疗效。结果:治疗组患者中医证候疗效优于对照组(P<0.05);治疗后两组患者中医证候积分均较治疗前降低(P<0.05),且治疗组患者治疗后中医证候积分低于对照组(P<0.05)。治疗组患者临床疗效优于对照组(P<0.05);治疗后两组患者TBIL、ALT均低于治疗前(P<0.05),ALB、PTA均高于治疗前(P<0.05),且治疗组患者治疗后TBIL、ALT均低于对照组(P<0.05),ALB、PTA均高于对照组(P<0.05)。结论:茵陈术附汤加减治疗乙型肝炎相关性慢加急性肝衰竭阴阳黄证具有较好临床疗效,能改善患者中医证候及肝功能。Objective:To observe the clinical efficacy of modified Yinchen Zhufu Decoction on hepatitis B related acute on chronic liver failure with Yin-Yang-huang syndrome.Methods:Totally 80 patients with hepatitis B related acute on chronic liver failure were divided into control and treatment groups,40 cases in each group,according to random number table.Patients were treated by comprehensive Western medicine in control group,and modified Yinchen Zhufu Decoction based comprehensive Western medicine in treatment group.The course of treatment was four weeks.The TCM syndrome scores,liver function(TBIL,ALT and ALB)and coagulase primary activity(PTA)were observed in the two groups before and after treatment,and the efficacy was evaluated.Results:The treatment group showed better TCM syndrome efficacy than control group(P<0.05).After treatment,TCM syndrome scores decreased in the two groups(P<0.05),and the treatment group showed lower TCM syndrome scores than control group(P<0.05).The treatment group showed better clinical efficacy than control group(P<0.05).After treatment,TBIL and ALT decreased in the two groups(P<0.05),while ALB and PTA increased in the two groups(P<0.05),and the treatment group showed lower TBIL and ALT than control group,while higher ALB and PTA than control group(P<0.05).Conclusion:Modified Yinchen Zhufu Decoction can provide good clinical efficacy on hepatitis B related acute on chronic liver failure with Yin-Yang-huang syndrome and improve TCM syndrome and liver function.
