上焦宣痹汤联合巴氯芬治疗原发性肝癌介入术后顽固性呃逆28例临床观察

摘　　要：目的:观察上焦宣痹汤联合巴氯芬治疗原发性肝癌介入术后顽固性呃逆的临床疗效及对患者生活质量的影响。方法:将60例原发性肝癌介入术后顽固性呃逆患者随机分为治疗组与对照组,每组30例。对照组患者口服巴氯芬片,治疗组患者在对照组基础上口服上焦宣痹汤,疗程为7 d。观察两组患者治疗前后呃逆症状评分,食欲、睡眠及精神状态3个方面的变化评分及中医证候评分,并评定临床疗效。结果:对照组剔除1例,脱落2例,最终完成27例;治疗组剔除1例,脱落1例,最终完成28例。治疗后治疗组患者呃逆症状评分及中医证候评分明显低于对照组,食欲、睡眠及精神状态评分明显高于对照组,差异均有统计学意义(P<0.05);治疗组临床疗效总有效率为92.86%(26/28),明显高于对照组的70.37%(19/27),差异有统计学意义(P<0.05)。结论:上焦宣痹汤联合巴氯芬可以明显改善原发性肝癌介入术后顽固性呃逆患者的临床症状及生活质量。Objective:To observe the clinical effect of Shangjiao Xuanbi Decoction combined with baclofen in the treatment of intractable hiccup after interventional operation of primary liver cancer and its influence on life quality.Methods:Totally 60 patients with intractable hiccup after interventional therapy for primary liver cancer,were randomly divided into treatment and control groups,30 cases in each group.The control group took baclofen tablets,while the treatment group took Shangjiao Xuanbi Decoction on the basis of the control group.The course of treatment was seven days.The hiccup symptom score,appetite score,sleep score,mental state score and TCM symptom score were observed in two groups before and after treatment and the clinical efficacy was evaluated.Results:In the control group,one case was removed and two cases were dropped;There were 27 cases finally completed.In the treatment group,one case was removed and one case was dropped;There were 28 cases finally completed.After treatment,the treatment group showed lower hiccup symptom score and TCM symptom score than control group,while higher appetite score,sleep score and mental state score than control group,with statistical significance(P<0.05).The total effective rate was 92.86%(26/28)in treatment group and 70.37%(19/27)in control group;The treatment group showed higher total effective rate than control group,with statistical significance(P<0.05).Conclusion:Shangjiao Xuanbi Decoction combined with baclofen can significantly improve the clinical symptoms and life quality of intractable hiccup after interventional therapy for primary liver cancer.
