杨素清中药序贯疗法治疗进行期银屑病的临床研究

摘　　要：目的:基于杨素清教授的“毒”“瘀”两极阻断学说模型,观察中药序贯疗法治疗临床寻常型进行期银屑病的疗效。方法:将银屑病患者随机分为治疗组和对照组,58例治疗组患者采用中药序贯疗法,前4周口服中药“解毒方”,后4周口服中药“化瘀方”;57例对照组患者口服消银胶囊,疗程为8周;比较两组患者治疗前、治疗4周、治疗8周后的银屑病皮损面积和严重程度指数(PASI)、中医证候评分、皮肤病生活质量指数(DLQI),分析“解毒方”和“化瘀方”的临床治疗效果。结果:在治疗前4周,“解毒方”在降低PASI评分、中医各证候评分及DLQI评分方面,疗效均优于消银胶囊(P<0.05);在治疗后4周,“化瘀方”在除口干、便干外的其余指标改善情况方面,疗效优于消银胶囊(P<0.05);在治疗的8周时间内,采用中药序贯疗法进行分期治疗,在改善各指标及整体临床疗效方面优于消银胶囊(P<0.05)。结论:“解毒方”配合“化瘀方”的序贯式治疗方法对银屑病的治疗具有较好的临床疗效,为运用“毒”“瘀”理论辨治银屑病提供了实践依据。Objective:To observe the therapeutic effect of Chinese medicine sequential therapy on psoriasis vulgaris based on Professor YANG Su-qing’s Two Poles Blocking Theory model of"Poison"and"Stasis".Methods:Patients with psoriasis were randomly divided into treatment and control groups.The 58 patients were treated with Chinese medicine sequential therapy in the treatment group,namely oral"Jiedu Prescription"(解毒方)at the first four weeks,and oral"Huayu Prescription"(化瘀方)at the next four weeks.The 57 patients took Xiaoyin Capsule(消银胶囊)orally in the control group for eight weeks.The PASI score,TCM syndrome score and DLQI score were evaluated respectively in the two groups,before treatment,four weeks after treatment and eight weeks after treatment.The efficacy of"Jiedu Prescription"and"Huayu Prescription"were analyzed.Results:"Jiedu Prescription"showed better effect on reducing PASI score,TCM syndrome score and DLQI score,than Xiaoyin Capsule at the first four weeks of treatment(P<0.05).In addition to the indicators of dry mouth and dry stool,"Huayu Prescription"showed better effect on other indexes than Xiaoyin Capsule at the next four weeks(P<0.05).Chinese medicine sequential therapy showed better effect on improving the indexes and overall clinical effect than Xiaoyin Capsule in the eight weeks of treatment(P<0.05).Conclusions:The sequential treatment method of"Jiedu Prescription"and"Huayu Prescription"shows a good effect on psoriasis,which provides a practical basis for the theories of"Poison"and"Stasis"in the differentiation and treatment of psoriasis.
