基于文献研究分析揿针在肿瘤辅助治疗中的应用现状及展望

摘　　要：目的探究揿针目前在肿瘤辅助治疗当中的应用特点,通过其取穴规律分析当前应用的优点与不足并提出展望。方法通过检索近10年揿针在恶性肿瘤辅助治疗中的中外临床研究文献,运用频数统计方法,对其临床应用特点及取穴规律进行统计分析。结果在所检索的18篇文献中,揿针在改善恶性肿瘤围手术期不适、改善化疗不良反应、改善临床症状等方面均有所应用,其中39%(7/18)的研究针对于化疗相关性恶心呕吐。各项研究的穴位选取中,有12篇文献选取足三里作为主穴,约占67%(12/18);9篇选取内关穴作为主穴,约占50%(9/18)。结论通过分析总结,现今揿针在肿瘤辅助治疗的多个方面均有应用,且临床疗效较为显著,其中关于化疗相关性恶心呕吐的研究占绝大多数,而揿针临床应用的取穴主要为足三里、内关等穴。揿针作用于这些穴位可发挥出长时间持续刺激的作用特点,但其相较于毫针刺法的高安全性特点并没有充分发挥,相反揿针刺激量相对较小的劣势却容易体现。故揿针在肿瘤辅助治疗中的优势仍有进一步发挥的空间,文章将对其发展进行初步展望。Objective To explore the current clinical application characteristics of the intradermal acupuncture in the adjuvant treatment of cancer, analyze the advantages and disadvantages of the current application through its acupoint selection rule, and propose a prospect.Methods By searching the literature of Chinese and foreign clinical research on the adjuvant treatment of cancer in the past 10 years, statistical analysis of its clinical application characteristics and acupuncture points was performed by using frequency statistics.Results Through the 18 documents retrieved, the intradermal acupuncture was used to improve the perioperative discomfort of cancer, the adverse effects of chemotherapy and clinical symptoms.Among them, 39%(7/18) of the studies focused on chemotherapy-induced nausea and vomiting.Among the acupoints selected in various studies, 12 articles selected Zushanli(ST36) as the main acupoint, accounting for 67%(12/18) and 9 articles selected Neiguan(PC6) as the main acupoint, approximately accounting for 50%(9/18).Conclusion According to the analysis and summary, intradermal acupuncture is used in many aspects of adjuvant cancer therapy, and the clinical effect is significant.Among them, the research on chemotherapy-induced nausea and vomiting accounts for the vast majority, and the main acupoints of clinical application of intradermal acupuncture are Zushanli(ST36) and Neiguan(PC6).The action of the intradermal acupuncture on these acupuncture points can exert the characteristics of long-term continuous stimulation, but its high safety characteristics compared to the filiform needle puncture have not been fully exerted.On the contrary, the relatively small amount of the intradermal acupuncture stimulation is easy to manifest.Therefore, the advantages of the intradermal acupuncture in the adjuvant treatment of cancer still have space for further development.This article made a preliminary outlook on its development.
