自制三黄紫草膏辅助治疗糖尿病足对创面愈合情况及血清5-HT、β-EP水平的影响观察

摘　　要：目的探讨自制三黄紫草膏辅助治疗糖尿病足对创面愈合情况及血清五羟色胺(5-HT)、β-内啡肽(β-EP水平)的影响。方法从2017年1月—2019年9月在该院接受治疗的糖尿病足患者中选出90例,按照治疗方法的不同分为对照组和观察组,每组45例,对照组采用常规换料,观察组在对照组的基础上给予自制三黄紫草膏辅助治疗,比较两组患者的创面愈合情况、治疗效果、肉芽生长情况、血清5-HT、β-EP水平。结果观察组的伤口面积缩小率、脱腐率、渗出减少率显著高于对照组,两组差异比较具有统计学意义(P<0.05);观察组治疗的总有效率为93.33%(42/45)显著高于对照组77.78%(35/45),两组差异比较具有统计学意义(P<0.05);两组肉芽覆盖达伤口床面积25%的时间无明显差异(P>0.05),覆盖面积达26%~50%、51%~75%、76%~100%的时间明显短于对照组,两组差异比较具有统计学意义(P<0.05);两组患者治疗前的5-HT、β-EP水平无明显差异(P>0.05),治疗后上述指标均上升,观察组显著高于对照组,两组差异显著具有统计学意义(P<0.05)。结论自制三黄紫草膏辅助治疗糖尿病足有助于创面的愈合,促进肉芽组织的生长,有效缓解疼痛,有助于患者的康复,值得临床推广。Objective To explore the effect of adjuvant therapy with self-made Sanhuang Zicao Ointment(三黄紫草膏) for diabetic foot on wound healing and levels of 5-hydroxytryptamine(5-HT) and β-endorphin(β-EP) in the serum.Methods Ninety patients with diabetic foot treated in the hospital between January 2017 and September 2019 were selected as the subjects.They were divided into the control group and the observation group according to the treatment method, with 45 cases in each group.Both groups were treated with conventional dressing change.On this basis, the observation group were given adjuvant therapy with self-made Sanhuang Zicao Ointment.The situation of wound healing, treatment effects, granulation growth and levels of 5-HT and β-EP were compared between the two groups.Results The reduction rate of wound area, slough shedding rate and reduction rate of exudation in the observation group were significantly higher than those in the control group(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(93.33%,42/45 vs 77.78%,35/45)(P<0.05).There was no significant difference in the time to granulation covering 25% of the wound bed area between the two groups(P>0.05).The time to the cover area reaching 26%-50%,51%-75% and 76%-100% in the observation group was significantly shorter than that in the control group(P<0.05).There was no significant difference in levels of 5-HT or β-EP between the two groups before treatment(P>0.05).The above indicators all increased after treatment, and they were significantly higher in the observation group than those in the control group(P<0.05).Conclusion The adjuvant therapy with self-made Sanhuang Zicao Ointment for diabetic foot can help wound healing, promote the growth of granulation tissues and effectively alleviate pain, which is conductive to patients′rehabilitation.
