中心静脉导管胸腔闭式引流术治疗结核性胸膜炎的临床效果观察

摘　　要：目的观察中心静脉导管胸腔闭式引流术治疗结核性胸膜炎的临床效果。方法将94例结核性胸膜炎患者按治疗方式不同分为对照组和观察组各47例。除常规抗结核治疗外,对照组采用胸腔穿刺抽液治疗,观察组采用中心静脉导管胸腔闭式引流术治疗,比较两组的临床指标、临床治疗效果及并发症发生情况。结果与对照组比较,观察组胸腔积液排出量较高,胸腔积液消失时间、退热时间、胸闷消失时间较短,胸膜厚度较低(P<0.05)。观察组治疗总有效率为93.62%,明显高于对照组的74.47%(P<0.05)。观察组的并发症发生率为4.26%,明显低于对照组的21.28%(P<0.05)。结论采用中心静脉导管胸腔闭式引流术治疗结核性胸膜炎的临床效果显著,且安全性较高,值得临床推广应用。Objective To observe the clinical effect of closed thoracic drainage with central venous catheter in the treatment of tuberculous pleurisy.Methods 94 patients with tuberculous pleurisy were divided into control group and observation group according to different treatment methods,with 47 cases in each group.In addition to routine anti-tuberculosis treatment,the control group was treated with thoracentesis,and the observation group was treated with closed thoracic drainage with central venous catheter.The clinical indicators,clinical treatment effects and complications were compared between the two groups.Results Compared with the control group,the pleural effusion discharge of the observation group was higher,the pleural effusion disappearance time,antipyretic time and chest tightness disappearance time were shorter,and the pleural thickness was lower(P<0.05).The total effective rate of treatment in the observation group was 93.62%,significantly higher than 74.47%of the control group(P<0.05).The incidence of complications in the observation group was 4.26%,significantly lower than 21.28%of the control group(P<0.05).Conclusions Closed thoracic drainage with central venous catheter in the treatment of tuberculous pleurisy has significant clinical effect and higher safety,which is worthy of clinical promotion and application.
