锁骨钩板塑形的规范化测量与应用效果

摘　　要：目的探讨CT、肩锁关节正位照片的规范化测量指导锁骨钩板塑形的应用效果。方法选取我院2010年1月至2019年12月收治的30例肩锁关节脱位患者,均行锁骨钩板内固定手术治疗。使用CT三维重建及肩锁关节正位照片来测量患者健侧肩锁关节高度、角度,并评估患者钩板植入前1 d、钩板植入后1周、钩板取出后的UCLA肩关节评分。结果CT三维重建与X-Ray测量肩锁关节高度及角度比较无统计学差异(P>0.05)。钩板取出后,患者的UCLA肩关节各项评分均高于钩板植入后1周(P<0.05);钩板植入后1周,患者的UCLA肩关节各项评分均高于钩板植入前1 d(P<0.05)。结论CT、肩锁关节正位照片的规范化测量指导锁骨钩板塑形的应用效果均较好,患者行锁骨钩板内固定手术治疗后肩关节恢复较好,可根据患者的具体情况选择合适的测量方式。Objective To explore the application effect of standardized measurement of CT and acromioclavicular anteroposterior photograph in the guidance of clavicular hook plate shaping.Methods 30 patients with acromioclavicular dislocation admitted to our hospital from January 2010 to December 2019 were treated by clavicular hook plate internal fixation.CT three-dimensional reconstruction and acromioclavicular anteroposterior photograph were used to measure the height and angle of uninjured-side acromioclavicular joints,and the UCLA shoulder scores at 1 day before hook plate implantation,1 week after hook plate implantation and after taking out the hook plate were evaluated.Results No statistical difference was found in the height and angle of acromioclavicular joints between X-Ray and CT three-dimensional reconstruction(P>0.05).After taking out the hook plate,the UCLA shoulder scores were higher than those at 1 week after hook plate implantation(P<0.05);1 week after hook plate implantation,the UCLA shoulder scores were higher than those at 1 day before hook plate implantation(P<0.05).Conclusions Standardized measurement of CT and acromioclavicular anteroposterior photograph in the guidance of clavicular hook plate shaping has good application effect,and patients have good recovery of acromioclavicular joints after clavicular hook plate internal fixation.Appropriate measurement can be chose according to specific situation of patients.
