摘  要:目的探讨神经内镜下小骨窗开颅微血管减压术治疗原发性三叉神经痛(PTN)的效果。方法60例PTN患者随机分为两组各30例,对照组采用显微镜下血管减压术治疗,观察组采用神经内镜下小骨窗开颅微血管减压术治疗。比较两组的治疗效果、并发症以及复发率。结果两组的治疗总有效率比较,差异无统计学意义(P>0.05)。观察组的术后并发症发生率为6.67%,显著低于对照组的26.67%(P<0.05)。随访2年,两组的复发率比较,差异无统计学意义(P>0.05)。结论神经内镜下小骨窗开颅微血管减压术治疗原发性三叉神经痛的效果好,术后并发症发生率及复发率低。Objective To explore the effect of small bone window craniotomy microvascular decompression under neuroendoscopy in the treatment of primary trigeminal neuralgia(PTN).Methods 60 patients with PTN were randomly divided into two groups,with 30 cases in each group.The control group was treated with vascular decompression under microscope,and the observation group was treated with small bone window craniotomy microvascular decompression under neuroendoscopy.The therapeutic effect,complication and recurrence rate were compared between two groups.Results No statistical difference was found between two groups in the total effective rate of treatment(P>0.05).The incidence of postoperative complications of the observation group was 6.67%,significantly lower than 26.67%of the control group(P<0.05).After 2 years of follow-up,no significant difference was found between two groups in the recurrence rate(P>0.05).Conclusions Small bone window craniotomy microvascular decompression under neuroendoscopy in the treatment of primary trigeminal neuralgia has good effect,low incidence of postoperative complications and low recurrence rate.
