摘 要:目的探讨高呼气末正压肺复张对严重脓毒症患者血流动力学的影响。方法选取我院2018年3月至2020年3月收治的60例严重脓毒症患者,随机分为两组各30例。观察组采用先压力控制通气(PCV)模式,后容量控制通气(VCV)模式治疗;对照组采用先VCV模式后PVC模式治疗。比较两组肺复张前以及肺复张后1 min、5 min、10 min的心率、平均动脉压(MAP)。结果肺复张前及肺复张后1 min,两组的心率和MAP无显著差异(P>0.05);肺复张后5 min、10 min,观察组的心率和MAP均显著高于对照组(P<0.05)。结论高呼气末正压肺复张能够显著提高严重脓毒症患者的血流动力学,确保患者机体运行效率。Objective To explore the impact of high positive end-expiratory pressure pulmonary reextension on the hemodynamics of patients with severe sepsis.Methods 60 patients with severe sepsis admitted to our hospital from March 2018 to March 2020 were selected and randomly divided into two groups,with 30 cases in each group.The observation group was treated with pressure controlled ventilation(PCV)mode,followed by volume controlled ventilation(VCV)mode.The control group was treated with VCV mode,followed by PCV mode.Before pulmonary reextension and 1 min,5 min and 10 min after pulmonary reextension,the heart rate and mean arterial pressure(MAP)were compared between two groups.Results Before pulmonary reextension and 1 min after pulmonary reextension,no statistical difference was found between two groups in the heart rate and MAP(P>0.05);5 min and 10 min after pulmonary reextension,the heart rate and MAP of the observation group were significantly higher than those of the control group(P<0.05).Conclusions High positive end-expiratory pressure pulmonary reextension can significantly improve the hemodynamics of patients with severe sepsis and ensure the operating efficiency of the body.
