加味芎归二陈汤联合屈螺酮炔雌醇片及维生素D胶囊对多囊卵巢综合征患者内分泌和糖脂代谢的影响

摘　　要：目的探讨加味芎归二陈汤联合屈螺酮炔雌醇片及维生素D胶囊对多囊卵巢综合征患者内分泌和糖脂代谢的影响;方法选取赣南医学院第一附属医院2019年1月—12月门诊治疗的多囊卵巢综合征患者180例,以随机数字表法分组,试验组90例采用加味芎归二陈汤联合屈螺酮炔雌醇片及维生素D胶囊治疗,对照组90例采用屈螺酮炔雌醇片联合维生素D胶囊治疗,6个月后随访,观察治疗前后激素水平[促黄体生成素(Luteinizing hormone,LH)、卵泡生成激素(Follicle stimulating hormone,FSH)、睾酮(Testosterone,T)]、糖脂代谢[总胆固醇(Total cholesterol,TC)、低密度脂蛋白胆固醇(Low-density lipoprotein cholesterol,LDL-C)、空腹血糖(Fasting blood-glucose,FBG)],以及治疗前后月经紊乱、闭经、肥胖的改善情况及不良反应。结果6个月后随访,2组LH、LH/FSH、T、TC、LDL-C、FBG水平均低于治疗前,且试验组优于对照组,差异有统计学意义(P<0.05);试验组月经紊乱、闭经发生率、体重指数低于对照组,差异有统计学意义(P<0.05);试验组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论加味芎归二陈汤联合屈螺酮炔雌醇片及维生素D胶囊用于临床治疗多囊卵巢综合征临床疗效显著,有利于改善月经紊乱、闭经情况,且能够促进内分泌及糖脂代谢平衡,药物不良反应较小,具有较高应用价值。Objective To investigate the effects of modified Xionggui Erchen decoction combined with drospirone ethinylestradiol tablets and vitamin D capsule on endocrine and glucose-lipid metabolism in patients with polycystic ovary syndrome(PCOS).Methods From January 2019 to December 2019,180 patients with PCOS in the First Affiliated Hospital of Gannan Medical University were selected.90 patients in the trial group were treated with modified Xionggui Erchen decoction combined with drospirone ethinylestradiol tablets and vitamin D capsule.90 patients in the control group were treated with tristroacetone estradiol tablet combined with vitamin D capsule.After six months followed-up,observation of hormone levels[Luteinizing hormone(LH),Follicle stimulating hormone(FSH),Testosterone(T)],carbohydrate-lipid metabolism[Total cholesterol]before and after treatment Cholesterol(TC),Low-density lipoprotein cholesterol(LDL-C),Fasting glucose,Fasting Blood glucose(FBG),and the improvement and adverse reactions of menstrual disorders,amenorrhea,obesity before and after treatment was made.Results After 6 months of follow-up,the levels of LH,LH/FSH,T,TC,LDL-C and FBG in 2 groups were lower than before treatment,and the experimental group was better than the control group,with statistical significance(P<0.05);The incidence of menstrual disorder,amenorrhea and body mass index in experimental group were lower than those in control group,with statistical significance(P<0.05).The incidence of adverse reactions in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion modified Xionggui Erchen decoction combined with drospirone ethinylestradiol tablets and vitamin D capsule is effective in clinical treatment of PCOS,which is beneficial to improve menstrual disorder and amenorrhea,and can promote the balance of endocrine and glycolipid metabolism.
