摘 要:目的探讨主动运动训练对肘关节骨折术后功能恢复的影响。方法选取2016年4月至2018年4月就诊于我院骨科的40例肘关节骨折术后患者,其中,男26例,女14例;年龄12~67岁,平均(40.00±5.30)岁。采用随机数字表法分为观察组和对照组,每组20例。两组患者均给予常规康复治疗,观察组在此基础上增加主动运动训练时间和频次,分别在术后3天和术后12周由同一康复医师对两组患者进行肘关节纽约特种外科医院(hospital for special surgery,HSS)功能评分和肘关节主动屈曲、伸直角度评定并记录。结果所有患者术后12周均获随访,未发生内固定松动、感染、异位骨化等不良并发症。术后12周,观察组HSS评分(92.45±3.87),对照组(80.65±6.10);观察组主动屈曲(120.75±9.07)°,对照组(106.00±5.28)°;观察组主动伸直(3.50±5.64)°,对照组(11.75±4.38)°;两组肘关节主动屈曲、伸直角度、HSS功能评分显著高于术后3天,组内术后12周与术后3天比较差异有统计学意义(P=0.000);且观察组的主动屈曲、伸直角度及HSS功能评分改善程度显著优于对照组,组间比较差异有统计学意义(P=0.000)。结论主动运动训练可提高肘关节骨折术后功能的恢复,增加肘关节主动活动度,有效地恢复肘关节术后功能。Objective To explore the effect of active exercise on the functional recovery after elbow fracture.Methods A series of 40 patients after elbow joint fracture were analyzed from April 2016 to April 2018,including 26 males and 14 females with an average of(40.00±5.30)years(range:12-67 years).Random number table was used to divide all patients into observation group(n=20)and control group(n=20).Observation group:regular conventional rehabilitation.Observation group:regular conventional rehabilitation plus increased time and frequency of active exercise.HSS and elbow functions were evaluated by the same physician 3 days and 12 weeks after surgery.Active flexion and extension angles were assessed and recorded.Results All patients were followed up for 12 weeks postoperatively.No internal fixation loosening,infection,or heterotopic ossification occurred.HSS,active flexion,and active extension of the observation group 12 weeks after surgery[(92.45±3.87),(120.75±9.07)°,(3.50±5.64)°]were significantly higher than that 3 days after surgery[(80.65±6.10),(106.00±5.28)°,(11.75±4.38)°](P<0.05).Conclusions Active exercise will improve elbow active range of motion and restore postoperative elbow joint functions effectively.
