摘 要:目的:分析胸膜活检术在胸腔积液的检测阳性率。方法:选取2018年10月-2020年1月在鹤山市人民医院接受治疗的胸腔积液患者50例作为研究资料,采取胸膜活检术。50例患者中,一次性活检成功36例(72.00%),两次活检成功11例(22.00%),三次活检成功3例(6.00%)。结果:在胸膜活检术中,三次活检共检出结核性胸膜炎10例(20.00%),恶性胸膜疾病21例(42.00%),恶性胸膜疾病中恶性胸膜间皮瘤3例(14.29%),腺癌7例(33.33%),大细胞癌1例(4.76%),小细胞未分化癌1例(4.76%),未定型癌7例(33.33%),鳞状细胞癌2例(9.52%);黏膜组织慢性炎检出3例(6.00%);横纹肌组织1例(2.00%)。共检出35例,可知胸膜活检术的检测阳性率为70.00%。其中第1次活检共检出30例,第1次阳性检出率为60.00%;第二次活检共检出33例,第二次阳性检出率为66.00%;第三次阳性检出35例,第三次阳性检出率为70.00%。结论:以胸膜活检术对胸腔积液患者进行检测,有着较高诊断准确率,同时可以采取反复检测的方式以提升检测阳性率,胸膜活检术在临床对胸腔积液患者的检测中具有临床推广价值。Objective:To analyze the positive rate of pleural biopsy in pleural effusion.Methods:50 patients with pleural effusion who were treated in Heshan People's Hospital from October 2018 to January 2020 were selected as research data,and pleural biopsy was performed.Among the 50 patients,36 cases were successful in one-time biopsy(accounting for 72.00%),11 cases were successful in two biopsies(accounting for 22.00%),and 3 cases were successful in three biopsies(accounting for 6.00%).Results:During the pleural biopsy,10 cases of tuberculous pleurisy(accounting for 20.00%),21 cases of malignant pleural disease(accounting for 42.00%),and 3 cases of malignant pleural mesothelioma(accounting for 42.00%)were detected in three biopsy procedures.Ratio 14.29%),7 cases of adenocarcinoma(accounting for 33.33%),1 case of large cell carcinoma(accounting for 4.76%),1 case of small cell undifferentiated carcinoma(accounting for 4.76%),7 cases of untyped cancer(accounting for 33.33%),2 cases of squamous cell carcinoma(accounting for 9.52%);3 cases of mucosal tissue chronic inflammation(accounting for 6.00%);1 case of striated muscle tissue(accounting for 2.00%).A total of 35 cases were detected,and the positive rate of pleural biopsy was 70.00%.A total of 30 cases were detected in the first biopsy,and the first positive detection rate was 60.00%;33 cases were detected in the second biopsy,the second positive detection rate was 66.00%,and the third positive detection rate was 35 Case;the third positive detection rate was 70.00%.Conclusion:Pleural biopsy for the detection of patients with pleural effusion has a high diagnostic accuracy.At the same time,repeated detection can be adopted to improve the positive rate of detection.Pleural biopsy has promotion value in the clinical detection of patients with pleural effusion.
