内镜注射治疗儿童原发性膀胱输尿管反流的研究进展

摘　　要：膀胱输尿管反流指尿液从膀胱到输尿管或肾脏的异常流动,主要由膀胱输尿管连接处的先天性发育异常所致。内镜注射是治疗膀胱输尿管反流的一种微创外科手术,通过向膀胱输尿管连接处注射填充材料,改变输尿管口的异常形态,从而达到抗反流的目的。内镜治疗具有较高的成功率,且创伤小、无需住院及留置导尿,而新的内镜注射技术、注射材料的出现和应用从整体上提高了内镜注射的成功率,因此内镜注射成为很多国外儿科医师的首选治疗方法,但在注射技术、注射材料以及组合的优先选择方面尚存争议,未来仍需要进行更多关于不同注射技术搭配不同注射材料的大样本长期随访研究,以进一步提高内镜注射的成功率和安全性。Vesicoureteral reflux is the abnormal flow of urine from the bladder to the ureter or kidney,which is mainly caused by congenital abnormalities in the vesicoureteral junction.Endoscopic injection is a minimally invasive surgical operation for the treatment of vesicoureteral reflux,which changes the abnormal shape of the ureteral orifice by injecting filler materials into the vesicoureteral junction,achieving the purpose of antireflux.Endoscopic injection has high success rate,small trauma,no need for hospitalization or indwelling catheterization.With the emergence and application of new endoscopic injection techniques and injection materials,the endoscopic injection success rate has improved in overall.Therefore,endoscopic injection has become the preferred treatment method of many foreign pediatricians,although there are still controversies over injection technology,injection materials and priority selection of combinations.In the future,more large-scale long-term follow-up studies on different injection techniques and different injection materials are still needed to further improve the success rate and safety of endoscopic injections.
