摘  要:目的探讨50 W功率射频消融的生物学效应。方法4头雄性实验猪在全麻下行开胸活体心外膜射频消融,按消融功率不同分为两组,即50 W组、35 W组,接触压力分别控制在5~15 g和5~25 g,两组均在不同消融指数(AI)指导下完成消融。比较消融损伤表面最大直径、最大深度及消融持续时间。结果心室外膜消融共产生66个消融损伤,其中50 W组不同AI值(500、400、350)指导下消融分别产生11、12、12个损伤点,35 W组分别产生11、10、10个损伤点;在所有消融点中,未发现有POP或焦痂产生。在50或30 W功率下,消融损伤表面最大直径、最大深度及消融持续时间均随AI值增加而增加(均P<0.05)。在AI=500、400指导下消融,50 W组产生的消融损伤表面最大直径均大于35 W组(均P<0.05),最大深度均小于35 W组(均P<0.05),消融持续时间均短于35 W组(均P<0.05);在AI=350的指导下消融,50 W组产生的消融损伤表面最大直径、最大深度均大于35 W组(均P<0.05),消融持续时间短于35 W组(P<0.05)。结论在AI=500和400时,与35 W消融比较,50 W消融产生的损伤面直径较大而深度较小。Objective To investigate the biological effect of radiofrequency ablation at a power of 50 W.Methods Four male experimental pigs underwent open chest epicardial radiofrequency ablation under general anesthesia.According to the radiofrequency power,they were divided into 50 W group and 35 W group,with contact force 5 to 15 g and 5 to 25 g,respectively.The ablation was performed under various ablation index(AI)in both groups.The maximum diameter and depth of ablation lesions and ablation duration were compared.Results A total of 66 lesions were formed in epicardial ablation,including 11,12 and 12 lesions in 50 W group,and 11,10 and 10 lesions in 35 W group,guided by three different AI(500,400 and 350),respectively.Pop and char were not observed in all ablation sites.The maximum lesion diameter and depth and ablation duration increased with the increase of AI(all P<0.05)in both groups.The maximum lesion diameter was larger,the maximum lesion depth was smaller and the ablation duration was shorter in 50 W group than in 35 W group both under AI 500 and 400(all P<0.05).Under AI 350,the maximum lesion diameter and depth were greater,and the ablation duration was shorter in 50 W group than in the 35 W group(all P<0.05).Conclusion Under AI 500 and 400,the ablation lesion diameter is lager and depth is smaller at a power of 50 W than 30 W.
