下鼻甲割治法治疗变应性鼻炎的临床疗效及安全性研究

摘　　要：目的评价下鼻甲割治法治疗变应性鼻炎(AR)的临床疗效及安全性。方法选取2017年8月至2020年1月成都中医药大学附属医院耳鼻喉科行下鼻甲割治法治疗的AR患者30例为研究对象,采用视觉模拟评分量表(VAS)、鼻眼结膜相关生活质量问卷调查量表(RQLQ)及下鼻甲肿胀程度评分为疗效评价指标;以出血、感染及粘连为安全性评价指标,分别对患者治疗前及治疗后1、4、8、12周的疗效及安全性进行比较。结果与治疗前相比,治疗后不同时间点患者的RQLQ积分及鼻阻VAS评分呈下降趋势,差异有统计学意义(P<0.05)。患者VAS总分、下鼻甲大小评分及喷嚏、流涕、鼻痒VAS评分在治疗后1、4、8、12周4个时间点分别与治疗前相比,以及治疗后1、4、8周两两比较,差异有统计学意义(P<0.05);治疗后8周与12周相比较,差异无统计学意义(P>0.05)。安全性评价方面,2例治疗后24 h内少量出血,3例在治疗1周后出现黏膜水肿及粘连,处理后均得到恢复,安全性较高。结论下鼻甲割治法治疗AR临床疗效确切,安全性较高。Objective To evaluate the clinical efficacy and safety of inferior turbinate cutting therapy in the treatment of allergic rhinitis(AR).Methods A total of 30 patients with AR who underwent inferior turbinate cutting therapy in the Department of Otorhinolaryngology,Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from August 2017 to January 2020 were selected as the research subjects.The efficacy and safety were compared before treatment and at 1,4,8,and 12 weeks after treatment,with visual analogue scale(VAS),rhinocoujunctivitis quality of life questionaire(RQLQ)and inferior turbinate swelling degree score as efficacy evaluation indexes,and bleeding,infection and adhesion as safety evaluation indexes.Results RQLQ score and VAS score of nasal obstruction at different time points after treatment showed a decreasing trend compared with those before treatment,and the difference was statistically significant(P<0.05).There were statistically significant differences in total VAS scores,inferior turbinate size scores,and the VAS scores of sneezing,runny nose and nasal itching before treatment and at each time point after treatment and in pairwise comparison at 1,4 and 8 weeks after treatment(P<0.05),while there was no significant difference between 8 weeks and 12 weeks after treatment(P>0.05).In terms of safety evaluation,2 cases suffered a small amount of bleeding within 24 h after treatment,and 3 cases developed mucosal edema and adhesion 1 week after treatment,which recovered after corresponding treatment.In general,this therapy is safe.Conclusion Inferior turbinate cutting therapy is effective and safe in the treatment of AR.
