原发性舍格伦综合征患者血浆弗林蛋白酶和Th1型细胞因子水平表达与唇腺损害及相关腺体功能的关系研究

摘　　要：目的探讨原发性舍格伦综合征(pSS)患者血浆弗林蛋白酶(FURIN)和辅助性T细胞1(Th1)型细胞因子水平表达与唇腺损害及相关腺体功能的关系。方法将邯郸市中心医院2017年8月~2019年8月收治的pSS患者109例作为pSS组,选取同期于邯郸市中心医院体检的健康志愿者90例作为对照组。比较两组血浆FURIN水平及Th1型细胞因子[白介素(IL)-2,IL-7,γ干扰素(INF-γ)]与Th2型细胞因子(IL-4,IL-13)水平。根据欧洲抗风湿联盟舍格伦综合征疾病活动指数(ESSDAI)将患者分成轻度组(n=37)、中度组(n=48)和重度组(n=24),比较三组上述各指标水平与唇腺损害及相关腺体功能指标(唾液流率、唇腺病理分级和Schirmer试验)水平。分析pSS患者抗SSA抗体、抗SSB抗体、血浆FURIN及Th1和Th2型细胞因子的关系,并观察血浆FURIN水平和Th1,Th2型细胞因子水平与唇腺损害及相关腺体功能指标的相关性。结果pSS组血浆FURIN及血清IL-2,IL-7,INF-γ,IL-4,IL-13水平均高于对照组(t=8.054~156.875,均P<0.05),Th1/Th2低于对照组(t=3.624,P=0.000)。中、重度组血浆FURIN及血清IL-2,IL-7,INF-γ,IL-4,IL-13水平、唇腺病理评分高于轻度组(t=5.339~36.092,均P<0.05),且重度组高于中度组(t=4.240~13.707,均P<0.05),而中、重度组Th1/Th2,唾液流率及左、右眼Schirmer试纸浸湿长度低于轻度组(t=3.013~42.715,均P<0.05),且重度组低于中度组(t=3.178~18.641,均P<0.05),差异均有统计学意义。抗SSA抗体、抗SSB抗体阳性患者的血浆FURIN水平及血清IL-2,IL-7,INF-γ,IL-4,IL-13水平高于阴性患者(t=2.560~14.333,均P<0.05),Th1/Th2低于阴性患者(t=2.363~4.604,均P<0.05),差异有统计学意义。血浆FURIN水平与血清IL-2,IL-7,INF-γ,IL-4,IL-13水平及唇腺病理评分呈正相关(r=0.362~0.640,均P<0.05),与Th1/Th2,唾液流率及左、右眼Schirmer试纸浸湿长度呈负相关(r=-0.715~-0.404,均P<0.05)。结论pSS患者血浆FURIN水平明显升高,且其水平变化和Th1及Th2型细胞因子�Objective To explore the relationship between plasma FURIN and Th1 type cytokines level,labial gland damage and related glandular function in patients with primary Sjogren’s syndrome(pSS).Methods A total of 109 patients with pSS who were admitted to Handan Central Hospital from August 2017 to August 2019 were included as the pSS group,and 90 healthy patients who received physical examination in Handan Central Hospital during the same period were selected as the control group.The levels of plasma FURIN and Th1 type cytokines[interleukin(IL)-2,IL-7,interferonγ(INF-γ)],Th2 type cytokines(IL-4,IL-13)were compared between the two groups.The patients were divided into mild group(n=37),moderate group(n=48)and severe group(n=24)according to the european league against rheumatism Sjgren’s syndrome disease activity index(ESSDAI).The above indexes level and labial gland damage and related glandular function indexes(saliva flow rate,labial gland pathological classification,Schirmer test)were compared in the three groups.Analyzed the relationship between plasma FURIN and Th1,Th2 type cytokines of pSS patients with anti-SSA and SSB antibodies,and observe the correlation between plasma FURIN level and Th1,Th2 type cytokines level,labial gland damage and related glandular function indexes.Results The levels of plasma FURIN,serum IL-2,IL-7,INF-γ,IL-4,IL-13 in the pSS group were higher than those in the control group(t=8.054~156.875,all P<0.05),Th1/Th2 was lower than that of the control group(t=3.624,P=0.000),the difference were stotistically significant,respectively.The levels of plasma FURIN,serum IL-2,IL-7,INF-γ,IL-4,IL-13 and labial gland pathology scores of the moderate and severe groups were higher than those of the mild group(t=5.339~36.092,all P<0.05),and the severe group was higher than the moderate group(t=4.240~13.707,all P<0.05),while the levels of Th1/Th2,saliva flow rate and the wetted length of Schirmer test paper in the left and right eyes of the moderate and severe group were lower than the mild group(t=3.
