腺样体切除联合鼓膜置管术治疗小儿难治性分泌性中耳炎的临床效果

摘　　要：目的探讨腺样体切除联合鼓膜置管术治疗小儿难治性分泌性中耳炎(SOM)的临床效果。方法纳入2016年6月至2018年12月35例小儿难治性SOM患儿,随机分组。鼓膜置管组选择鼓膜置管术,鼓膜置管+切除腺样体组选择腺样体切除联合鼓膜置管术。比较两组听力、炎症指标、中耳症状消失时间、听力改善时间、住院时间、并发症发生率。结果鼓膜置管+切除腺样体组听力、超敏C反应蛋白、白细胞介素-6明显优于鼓膜置管组,P<0.05;鼓膜置管+切除腺样体组中耳症状消失时间、听力改善时间、住院时间均短于鼓膜置管组,并发症发生率低于鼓膜置管组,P<0.05。结论小儿难治性SOM患儿实施腺样体切除联合鼓膜置管术可获得较好效果。Objective To investigate the clinical effect of adenoidectomy combined with tympanostomy in the treatment of refractory secretory otitis media with pediatric effusion.Methods A total of 35 children with refractory secretory otitis media from June 2016 to December 2018 were enrolled in the study.The tympanic membrane tube group was selected by tympanic membrane catheterization,and the tympanic membrane tube+adenoidectomy group was selected for adenoidectomy combined with tympanic membrane placement.Comparing the two groups'hearing,inflammation indicators,disappearance time of middle ear symptoms,hearing improvement time,hospitalization time,and complication rate.Results The hearing,high-sensitivity C-reactive protein,and interleukin-6 in the tympanic tube+adenoidectomy group were significantly better than those in the tympanic membrane tube group,P<0.05;the time to disappearance of middle ear symptoms and hearing in the tympanic membrane tube+adenoidectomy group the improvement time and hospitalization time were shorter than those of the tympanic membrane tube group,and the complication rate was lower than that of the tympanic membrane tube group,P<0.05.Conclusion Adenoidectomy combined with tympanostomy in children with refractory secretory otitis media with effusion can achieve better results.
