补硒联合左甲状腺素钠对甲状腺功能减退症患者甲状腺自身抗体水平的影响

摘　　要：目的探讨补硒联合左甲状腺素钠治疗甲状腺功能减退症的临床疗效。方法选取2017年7月至2019年7月本院收治的86例甲状腺功能减退症患者,根据随机数字表法分为观察组和对照组,每组43例。对照组采用左甲状腺素钠治疗,观察组在对照组基础上加用硒酵母治疗。比较两组甲状腺自身抗体水平、甲状腺功能及不良反应情况。结果治疗后,观察组甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)、促甲状腺激素(TSH)水平均低于对照组,游离三碘甲腺原氨酸(FT3)、游离四碘甲腺原氨酸(FT4)水平均高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义。结论补硒联合左甲状腺素钠可改善甲减患者的甲状腺功能,降低甲状腺抗体水平,且安全性高,值得临床推广应用。Objective To investigate the clinical efficacy of selenium supplementation combined with levothyroxine sodium in the treatment of hypothyroidism.Methods 86 patients with hypothyroidism admitted to our hospital from July 2017 to July 2019 were selected and divided into ob-servation group and control group according to the random number table method,with 43 cases in each group.The control group was treated with le-vothyroxine sodium,and the observation group was treated with selenium yeast on the basis of the control group.The levels of thyroid autoantibody levels,thyroid function and adverse reactions between the two groups were compared.Results After treatment,the levels of thyroglobulin antibody(TGAb),thyroid peroxidase antibody(TPOAb),and thyroid-stimulating hormone(TSH)in the observation group were lower than those in the con-trol group.The level of free triiodothyronine(FT3),free tetraiodothyronine(FT4)were higher than that of the control group,the difference was statisti-cally significant(P<0.05);there was no significant difference in adverse reactions between the two groups.Conclusion Selenium supplementation combined with levothyroxine sodium therapy can improve the thyroid function of patients with hypothyroidism,reduce the level of thyroid antibod-ies,and the safety is high,which is worthy of clinical application.
