喉罩全麻联合超声引导下改良髂筋膜间隙阻滞在老年股骨近端骨折麻醉中的应用研究

摘　　要：目的喉罩全麻联合超声引导下改良髂筋膜间隙阻滞在老年股骨近端骨折麻醉中的应用效果。方法选取2019年10月至2020年10月本院收治的80例股骨近端骨折老年患者,根据治疗方法的不同分为对照组和观察组,每组40例。对照组采用喉罩全麻,观察组采用在对照组基础上超声引导下改良髂筋膜间隙阻滞。比较两组患者手术麻醉效果。结果观察组麻药使用剂量、相关麻醉时间指标均显著优对照组(P<0.05);两组患者置入人工气道即刻心率、收缩压均发生了明显的波动,对照组在手术开始5 min后收缩压明显升高(P<0.05);术后2、6、12 h组间视觉模拟疼痛评分(VAS)比较差异有统计学意义(P<0.05)。结论喉罩全麻联合超声引导下改良髂筋膜间隙阻滞可明显提高麻醉效率,安全性较高,值得临床推广运用。Objective To investigate the effect of laryngeal mask general anesthesia combined with ultrasound ultrasound-guided improved iliac fascia space block in anesthesia of senile proximal femoral fracture.Methods 80 elderly patients with proximal femoral fracture admitted to our hospi-tal from October 2019 to October 2020 were selected and divided into control group and observation group according to different treatment methods,with 40 cases in each group.The control group received laryngeal mask general anesthesia,and the observation group received laryngeal mask general anesthesia combined with ultrasonic guided improved iliac fascia space block.The anesthesia effect was compared between the two groups.Results The anesthetic dosage and related anesthesia time in the observation group were significantly better than those in the control group(P<0.05).In both groups,the immediate heart rate and systolic blood pressure fluctuated significantly,while in the control group,the systolic blood pressure increased significantly 5 min after operation(P<0.05).There were statistically significant differencein visual analogue pain score(VAS)between groups at 2,6 and 12 h after surgery(P<0.05).Conclusion Laryngeal mask anesthesia combined with ultrasound guided improved iliac fascia space block can significantly improve the anesthetic efficiency,high safety,worthy of clinical application.
