摘  要:目的建立并评价一种新的肺动脉高压(PAH)合并肺缺血再灌注损伤(LIRI)大鼠模型。方法将40只雄性SD大鼠随机分为4组(n=10):对照组(Control),肺动脉高压组(PAH),肺缺血再灌注组(IR),肺动脉高压合并肺缺血再灌注组(PAH+IR)。PAH组大鼠以腹腔注射野百合碱方法建立PAH模型,IR组采用血管夹夹闭肺门法构建LIRI模型,PAH+IR组大鼠注射野百合碱4周后,应用血管夹夹闭肺门法建立PAH合并LIRI大鼠模型。检测各组大鼠平均肺动脉压(mPAP)、右心室收缩压(RVSP)、右心室肥厚指数(RVHI)、肺湿干重比值(W/D),并通过肉眼及光镜观察肺组织变化。结果PAH+IR组大鼠的mPAP、RVSP、RVHI均显著高于对照组和IR组(均P<0.05),PAH+IR组大鼠的mPAP及RVSP明显高于PAH组(均P<0.05);光镜下PAH组和PAH+IR组可见血管内皮层增生,肺小血管闭塞,IR组和PAH+IR组可观察到肺损伤改变,且PAH+IR组肺损伤较IR组明显;IR组和PAH+IR组大鼠的左肺W/D均显著高于对照组和PAH组(均P<0.05)。结论PAH和LIRI相互影响,促进疾病进展。该研究成功建立PAH合并LIRI大鼠模型,操作简便、成功率高,为研究PAH合并LIRI的病因及防治提供了可靠动物模型。Objective To establish and evaluate a model of pulmonary artery hypertension(PAH)complicated with lung ischemia-reperfusion injury(LIRI)in rats.Methods Forty SD rats were randomly divided into four groups(n=10):control group,PAH group,lung ischemia-reperfusion group(IR)and PAH with lung ischemia-reperfusion group(PAH+IR).The model of PAH was established by intraperitoneal injection of monocrotaline.The model of LIRI was established by clamping the pulmonary hilum.The model of PAH+IR was established by clamping the pulmonary hilum 4 weeks after the injection of monocrotaline.Mean pulmonary artery pressure(mPAP),right ventricular systolic pressure(RVSP),right ventricular hypertrophy index(RVHI)and lung wet-to-dry weight ratio(W/D)were measured in each group and changes in lung tissue were observed by naked eye and light microscope.Results mPAP,RVSP and RVHI in PAH+IR group were higher than those in Control and IR groups(all P<0.05).mPAP and RVSP in PAH+IR group were higher than those in PAH group(all P<0.05).There were vascular endothelial proliferation and small vessel occlusion in PHA and PAH+IR groups under microscope.Pathological changes of lung injury were observed in IR and PAH+IR groups,which were more obvious in PAH+IR group than in IR group.W/D was significantly higher in IR and PAH+IR groups than in Control and PAH groups(all P<0.05).Conclusion PAH and LIRI influence each other and promote disease progression.We successfully established a model of PAH complicated with LIRI in rats with simplicity of operation and high success rate,which could be used as an appropriate animal model for research of etiology,prevention and treatment of PAH complicated with LIRI.
