摘 要:目的探讨免疫球蛋白治疗儿童脓毒症的临床效果及对免疫功能的影响。方法选取2019年5月—2020年4月肇庆市第一人民医院儿科收治的40例脓毒症患儿为研究对象,按照随机抽签法分为对照组(20例)和干预组(20例),对照组进行常规对症治疗,干预组在对照组基础上增加免疫球蛋白治疗,比较两组的治疗效果及治疗前后免疫球蛋白(IgG、IgA、IgM),补体(C3、C4)的水平变化。结果干预组治疗总有效率为90.00%,高于对照组的60.00%,差异有统计学意义(P<0.05)。两组治疗前的IgG、IgA、IgM的水平比较,差异无统计学意义(P>0.05);治疗后干预组的IgG水平高于治疗前,且高于对照组,差异有统计学意义(P<0.05),而两组治疗后的IgA、IgM水平比较,差异无统计学意义(P>0.05)。两组治疗前C3、C4水平比较,差异无统计学意义(P>0.05);两组治疗后的C3、C4水平低于治疗前,且干预组的C3、C4水平低于对照组,差异有统计学意义(P<0.05)。结论免疫球蛋白治疗儿童脓毒症的临床效果显著,能有效改善免疫功能与患儿的临床症状,促进患儿康复,值得推广应用。Objective To investigate the clinical effect of immunoglobulin in the treatment of children with sepsis and its effect on immune function. Methods A total of 40 children with sepsis treated in the Department of Pediatrics of Zhaoqing First People′s Hospital from May 2019 to April 2020 were selected as the research objects, and divided into control group(20 cases) and intervention group(20 cases) according to random drawing method. The control group received conventional symptomatic treatment, and the intervention group received additional immunoglobulin treatment on the basis of the control group. The therapeutic effect and the levels of immunoglobulin(IgG, IgA, IgM) and complement(C3, C4) before and after treatment were compared between the two groups. Results The total effective rate of the intervention group was 90.00%, which was higher than that of the control group, the difference was statistically significant(P<0.05). There was no significant difference in the levels of IgG, IgA and IgM between the two groups before treatment(P>0.05). After treatment, IgG levels in the intervention group were higher than before treatment and higher than those in the control group, the differences were statistically significant(P<0.05). There was no significant difference in IgA and IgM levels between the two groups after treatment(P>0.05). Before treatment, there was no significant difference in C3 and C4 levels between the two groups(P>0.05). After treatment, the levels of C3 and C4 in the two groups were lower than before treatment, and the levels of C3 and C4 in the intervention group were lower than those in the control group, the differences were statistically significant(P<0.05). Conclusion Immunoglobulin in the treatment of sepsis in children has significant clinical effect, can effectively improve the immune function and clinical symptoms of children,promote the recovery, it is worthy of popularization and application.
