摘  要:肠道菌群及其代谢物参与了各种心血管疾病的发病过程以及预后进展,尤其是在心力衰竭的发生发展过程中发挥着重要的作用。本文主要讨论了一种肠道菌群代谢物--氧化三甲胺(TMAO)参与心力衰竭的病理机制。首先,TMAO可以通过介导炎症反应加重心衰;其次,TMAO可以通过相关信号通路加重心肌肥厚恶化心衰,并且TMAO可以参与细胞内T小管的重塑干扰心肌的收缩功能;最后,TMAO也可通过间接途径加重心衰。与此同时,TMAO可以作为一种早期预警标志物来监测心衰患者的恶化风险,对心力衰竭的危险分层及诊治具有重要价值,为心力衰竭防治提供了新靶点。Gut microbiome are involved in the pathogenesis and prognosis of various cardiovascular diseases, and play an important role in the occurrence and development of heart failure. In this review, we focus on one of the intestinal microbiota metabolites, trimethylamine oxide(TMAO), in the pathogenesis of heart failure. Firstly, TMAO can aggravate heart failure by inflammatory response. Secondly, TMAO can aggravate cardiac hypertrophy and worsen heart failure through signaling pathways. Moreover, TMAO can participate in the remodeling of intracellular T tubules and interfere with the systolic function of myocardium. Finally, TMAO can aggravate heart failure through indirect pathways. At the same time, it was reported that TMAO could be used as an early warning marker to monitor the people who was at risk of heart failure progression, which had important value for the risk stratification, diagnosis and treatment of heart failure, and provided a new target for the prevention and treatment of heart failure.
