摘　　要：目的了解南京市近5年各级医疗机构消毒现状,为提高医疗机构的消毒质量和感染控制能力提供参考。方法对各级医疗机构的重点科室进行现场随机采样和检测。结果 5年共采样24 107份,合格23 461份,总体合格率为97.32%,2015—2019年各年份合格率依次为96.27%、97.37%、97.27%、97.26%和98.30%,总体呈上升趋势,差异有统计学意义(χ^(2)_(趋势)=16.723,P<0.01)。三级、二级、一级和无级别医疗机构合格率依次为99.72%、96.95%、95.04%和97.32%,差异有统计学意义(χ^(2)_(趋势)=199.130,P<0.01)。医院污水合格率逐年提高,差异有统计学意义(χ^(2)_(趋势)=9.639,P<0.01);紫外线灯合格率逐年降低,差异有统计学意义(χ^(2)_(趋势)=9.080,P<0.01)。结论连续5年各级医疗机构消毒质量总体良好,合格率总体上呈逐年增高趋势且与医疗机构级别呈正相关,但还需继续加强各级医疗机构,特别是一级和无级别医疗机构医院污水和紫外线灯的消毒管理。Objective To know the disinfection status of medical institutions at all levels in Nanjing over the past five years, so as to provide basis for improving disinfection quality of medical institutions and infection control in hospitals. Methods Random sample and detection were used to monitor environmental infectious factors of medical institutions at all levels in Nanjing.Results A total of 24 107 samples were monitored for 5 consecutive years,among which 23 461 were qualified. The total qualified rate was 97.32%, the qualified rates from 2015 to 2019 were 96.27%, 97.37%, 97.27%, 97.26% and 98.30% respectively, showing an upward trend, and the difference was statistically significant(χ^(2) linear-by-linear =16.723, P<0.01). The qualified rate of hospital wastewater was increasing over the past five years, and the difference was statistically significant(χ^(2) linear-by-linear =9.639, P<0.01). The qualified rate of ultraviolet lamp intensity was getting worse over the past five years, and the difference was statistically significant(χ^(2) linear-by-linear =9.080, P<0.01). Conclusion The overall qualified rate of medical institutions at different levels from 2015 to 2019 is good. The qualified rate gets higher trend over the past five years and is positively correlated with the levels of medical institutions. The monitor and management of disinfection should be further strengthened for hospital wastewater and ultraviolet lamp, especially in primary hospital and non-hierarchical hospital.
