摘 要:目的使用Meta分析评价血管闭合装置在神经介入诊疗术后的应用效果。方法遵循循证医学的原则,采用计算机检索EMbase、Medline、Cochrane Library、CNKI、万方、维普等数据库,检索时限为2000年1月至2020年10月。手工检索补充,以检索有关血管闭合装置在神经介入诊疗术后应用效果的文献,由2名评价者共同评价所纳入文献的质量。应用RevMan5.4.1软件进行Meta分析及数据处理。结果共纳入13篇文献,包括3741例患者。Meta分析结果显示:①血管闭合装置组和人工压迫组的术后止血成功率无显著差异(P=0.39);②血管闭合装置组和人工压迫组的术后血管相关并发症发生情况比较,差异具有统计学意义(P<0.00001);③血管闭合装置组和人工压迫组的术后止血时间比较,差异具有统计学意义(P<0.00001);④血管闭合装置组和人工压迫组的术后制动时间比较,差异具有统计学意义(P<0.00001)。结论神经介入诊疗术后应用血管闭合装置安全、有效,可显著减少术后血管相关并发症,缩短止血及制动时间。Objective To evaluate the application effect of vascular closure device after neurointerventional therapy by Meta-analysis.Methods According to the principle of evidence-based medicine,the databases of EMbase,Medline,Cochrane library,CNKI,Wanfang and VIP were searched by computer,and the retrieval time limit was from January 2000 to October 2020.Manual search supplement was used to search the literature about the application effect of vascular closure device after neurointerventional therapy.The quality of literatures were evaluated by two evaluators.The Meta-analysis and data processing were carried out by RevMan5.3 software.Results A total of 13 literatures were included,including 3741 patients.The results of Meta-analysis showed that:①there was no significant difference in the success rate of postoperative hemostasis between the vascular closure device group and the artificial compression group(P=0.39);②the difference of postoperative vascular complications between the vascular closure device group and the artificial compression group was statistically significant(P<0.00001);③the difference of postoperative hemostasis time between the vascular closure device group and the artificial compression group was statistically significant(P<0.00001);④the difference of postoperative braking time between the vascular closure device group and the artificial compression group was statistically significant(P<0.00001).Conclusion Application of vascular closure device after neurointerventional therapy is safe and effective,which can significantly reduce postoperative vascular complications and shorten the hemostasis time and braking time.
