摘　　要：目的比较不同出生胎龄围产新生儿同步12导联心电图肢体QRS波振幅,探讨肢体导联QRS波振幅与出生胎龄的相关性,为围产新生儿心电图诊断提供参考依据。方法回顾性分析不同胎龄围产新生儿的肢体导联心电图,根据出生胎龄不同将其分为A组(胎龄30~<33周,64例)、B组(胎龄33~<37周,72例)、C组(胎龄37~40周,102例)。比较三组的肢体导联Q、R、S波振幅;分析各肢体导联QRS波群主波与出生胎龄相关性。结果随着出生胎龄增加,SⅠ、SaVL、QⅡ、QⅢ、QaVF、QaVR波逐渐加深,RⅡ、RⅢ、RaVF、RaVR波逐渐增高。Pearson相关性分析结果显示,SⅠ、SaVL、RⅡ、RⅢ、RaVF、RaVR振幅与出生胎龄呈显著线性正相关(P<0.001)。结论围产新生儿肢体导联QRS波振幅随着出生胎龄的增加而增加,QRS波振幅与出生胎龄的相关性较显著。Objective To compare the limb QRS wave amplitude in 12 lead synchronous electrocardiogram of perinatal newborns with different gestational age,and to explore the correlation between limb leads QRS wave amplitude and gestational age,so as to provide reference for electrocardiogram diagnosis of perinatal newborns.Methods The limb lead electrocardiogram of perinatal newborns with different gestational age were retrospectively analyzed.According to different gestational age,the newborns were divided into group A(gestational age 30-<33 weeks,64 cases),group B(gestational age 33-<37 weeks,72 cases)and group C(gestational age 37-40 weeks,102 cases).The amplitudes of Q,R and S waves of limb leads in the three groups were compared;the correlation between the main wave of QRS complex of limb lead and gestational age was analyzed.Results With the increase of gestational age,SⅠ,SaVL,QⅡ,QⅢ,QaVF,QaVR gradually deepened,RⅡ,RⅢ,RaVF,RaVR gradually increased.Pearson correlation analysis results showed that the amplitudes of SⅠ,SaVL,RⅡ,RⅢ,RaVF,RaVR were positively correlated with gestational age(P<0.001).Conclusion The amplitude of QRS wave in limb leads of perinatal newborns increases with the increase of gestational age,and the correlation between QRS wave amplitude and gestational age is significant.
