摘  要:脓毒症(Sepsis)是宿主对感染的反应失调,并产生危及生命的器官功能障碍[1],常发生在烧伤、感染、多发伤等临床急危重疾病的基础上,也是导致多器官功能障碍综合征(MODS)的重要原因之一[2]。脓毒症诱导的心肌抑制(Sepsis—induced myocardial dysfunction,SIMD)是严重脓毒症和脓毒性休克常见的并发症之一[3],是导致脓毒症多器官功能障碍及其病死率增加的重要因素。左西孟旦是首个获批用于治疗重度心力衰竭的钙增敏剂,目前已广泛应用于冠状动脉粥样硬化性心脏病心力衰竭患者的治疗[4],在脓毒症中也得到一定的应用[3]。动物实验表明,左西孟旦有改善脓毒症动物心功能、降低外周血管阻力、扩张肺动脉、改善胃肠功能等的作用[6]。由于肌丝钙敏感性降低在实验性脓毒性心肌病中已被反复证实[7],因此推测钙增敏剂也可能改善脓毒血症的心肌功能障碍。然而,钙增敏剂在脓毒性心肌病中的作用尚未得到彻底研究。因此,本文就左西孟旦对脓毒性心肌病动物模型心功能机制的研究进展作一综述。Sepsis is the maladjustment of the host response to infection,which leads to life-threatening organ dysfunction.Sepsis often occurs on the basis of clinical acute and critical diseases such as burns,infections and multiple injuries,and it is also one of the important reasons leading to multiple organ dysfunction syndrome(MODS)[2].Sepsisinduced myocardial dysfunction(SIMD)is one of the common complications of severe sepsis and septic shock[3].It is an important factor leading to multiple organ dysfunction and increased mortality in sepsis.Levosimendan is the first calcium sensitizer approved for the treatment of severe cardiac failure,which has been widely used in the treatment of patients with coronary heart disease and cardiac failure and has also been used in sepsis.Animal experiments show that levosimendan can improve cardiac function,reduce peripheral vascular resistance,dilate pulmonary artery and improve gastrointestinal function in septic animals.Because the decrease of myofilament calcium sensitivity has been repeatedly confirmed in experimental septic cardiomyopathy,it is speculated that calcium sensitizers may also improve myocardial dysfunction caused by sepsis.However,the role of calcium sensitizer in septic cardiomyopathy has not been thoroughly researched.Therefore,the research progress of levosimendan on cardiac function mechanism of septic cardiomyopathy animal model was reviewed in this paper.
