摘  要:目的分析姜黄素通过p38-丝裂原活化的磷酸激酶(p38MAPK)/核因子κB(NF-κB)信号通路对膝关节骨性关节炎的作用机制。方法采用前瞻性研究,随机抽样法选取2018年1月至2019年12月期间在邯郸市中心医院进行全膝关节置换术的35例膝关节骨性关节炎患者作为研究对象,取截掉的股骨髁和胫骨平台的软骨组织,将其分为模型组、低剂量组、中剂量组和高剂量组。并选取同期因外伤在邯郸市中心医院骨关节外科或血管外科行股骨截肢的47例患者作为对照组,取其膝关节健康软骨。模型组与对照组在不含姜黄素的培养基内传代2代细胞,低剂量组、中剂量组和高剂量组的软骨组织细胞分别在含有100、150、200 mg/kg姜黄素的培养基内传代2代细胞。对各组腺苷酸环化酶相关蛋白1(CAP1)、p38MAPK、p65-NF-κB及其磷酸化表达情况进行比较,并对各组白细胞介素(IL)-1β、IL-6、IL-8、IL-1β、肿瘤坏死因子-α(TNF-α)表达情况进行比较。结果与对照组相比,模型组的CAP1、p38MAPK、p65-NF-κB及其磷酸化表达水平均显著升高,差异均有统计学意义(P<0.05);与模型组相比,低剂量组、中剂量组、高剂量组的CAP1、p38MAPK、p65-NF-κB及其磷酸化表达水平均降低,且随着姜黄素剂量的升高而降低,差异均有统计学意义(P<0.05)。与对照组相比,模型组的IL-6、IL-8、IL-1β、TNF-α表达水平均显著升高,差异均有统计学意义(P<0.05);与模型组相比,低剂量组、中剂量组、高剂量组的IL-6、IL-8、IL-1β、TNF-α表达水平均显著降低,且随着姜黄素剂量的升高而降低,差异均有统计学意义(P<0.05)。结论通过激活p38MAPK/NF-κB信号通路姜黄素可调节膝关节骨性关节炎的炎症反应过程,且呈现明显的剂量反应。Objective To analyze the mechanism of curcumin on knee osteoarthritis through p38-mitogen-activated phosphokinase(p38MAPK)/nuclear factor-κB(NF-κB)signaling pathway.Methods In this prospective study,35 patients with knee osteoarthritis who underwent total knee arthroplasty in Handan Central Hospital from January 2018 to December 2019 were selected by random sampling method.The cartilage tissues of femoral condyle and tibial plateau were removed and divided into model group,low-dose group,medium dose group and high-dose group.Meanwhile,47 cases of femoral amputation due to trauma in the Department of osteoarthritis or vascular surgery of Handan central hospital were selected as the control group.The chondrocytes of model group were passaged for 2 generations in the medium without curcumin,and the chondrocytes of low-dose group,medium dose group and high-dose group were passaged for 2 generations in the medium containing 100,150 and 200 mg/kg curcumin,respectively.The levels of adenylate cyclase-related protein 1(CAP1),p38MAPK,p65-NF-κB and their phosphorylation expression in each group,and interleukin(IL)-1β,IL-6,IL-8,IL-1β,and tumor necrosis factor-α(TNF-α)expressions were compared.Results Compared with the control group,the levels CAP1,p38MAPK,p65-NF-κB and its phosphorylation expression in the model group were significantly increased,and the differences were statistically significant(P<0.05);compared with the model group,the levels of CAP1,p38MAPK,p65-NF-κB and its phosphorylation expression in the low-dose,middle and high dose groups were decreased,and with the increase of curcumin dose decreased,the differences were statistically significant(P<0.05).Compared with the control group,the levels of IL-6,IL-8,IL-1β,and TNF-αexpression in the model group were significantly increased,and the differences were statistically significant(P<0.05);compared with the model group,the levels of IL-6,IL-8,IL-1β,and TNF-αexpression in the low dose,middle-dose,and high-dose group were significantly decreased,and wit
