摘 要:目的探讨原发性甲状腺功能减低症患儿血清25羟维生素D3[25(OH)D3]及瘦素水平变化,及其与甲状腺功能的相关性。方法回顾性选取2016年1月至2019年12月在湖北省妇幼保健院治疗的原发性甲状腺功能减低症患者97例纳入研究组,按照维生素D营养状况将研究组分为维生素D缺乏组(9例),维生素D不足组(15例),维生素D充足组(73例),并选取同期体检的健康儿童97例作为对照组。分析各组患者血清25(OH)D3、瘦素水平变化,及其与甲状腺功能指标[游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)、甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)]的相关性。结果研究组患者的血清25(OH)D3水平为(67.93±21.16)nmol/L,明显低于对照组[(79.99±23.03)nmol/L],血清瘦素水平为(26.22±8.52)ng/mL,明显高于对照组[(9.96±2.16)ng/mL],差异均有统计学意义(P<0.05)。维生素D缺乏组、维生素D不足组患儿的血清25(OH)D3、TPOAb水平明显低于维生素D充足组,差异有统计学意义(P<0.05);维生素D缺乏组、维生素D不足组血清瘦素、TSH水平高于维生素D充足组,差异有统计学意义(P<0.05);不同维生素D营养状况患儿的血清FT3、FT4、TgAb水平比较,差异无统计学意义(P>0.05)。经Pearson分析,血清25(OH)D3与瘦素呈负相关(r=-0.589,P<0.001);血清25(OH)D3与FT3、FT4、TgAb、TPOAb呈正相关(P<0.05),与TSH呈负相关(P<0.05);血清瘦素与TSH呈正相关(P<0.05),与TPOAb呈负相关(P<0.05)。结论原发性甲状腺功能减低症患儿血清25(OH)D3呈低表达,瘦素呈高表达,血清25(OH)D3、瘦素与甲状腺功能有关,与原发性甲状腺功能减低密切相关。Objective To investigate the changes of serum 25-hydroxyvitamin D3[25(OH)D3]and leptin levels in children with primary hypothyroidism and their correlation with thyroid function.Methods A total of 97 patients with primary hypothyroidism treated in Hubei Maternal and Child Health Hospital from January 2016 to December 2019 were retrospectively selected and included in the study group.According to the nutritional status of vitamin D,the study group was divided into vitamin D deficiency group(9 cases),vitamin D deficiency group(15 cases),vitamin D sufficient group(73 cases).A total of 97 healthy children during the same period were selected as the control group.The changes of serum 25(OH)D3,leptin levels,and their thyroid function indicators[free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH),thyroglobulin antibody(TGAb),thyroid peroxidase antibody(TPOAb)]were analyzed.Results The level of serum 25(OH)D3 in the study group was(67.93±21.16)nmol/L,which was significantly lower than that in the control group[(79.99±23.03)nmol/L],and the serum leptin level was(26.22±8.52)ng/mL,which was significantly higher than that in the control group[(9.96±2.16)ng/mL],the differences were statistically significant(P<0.05).The levels of serum 25(OH)D3 and TPOAb in the vitamin D deficiency group and the vitamin D deficiency group children were significantly lower than those in the vitamin D sufficient group,and the differences were statistically significant(P<0.05).The levels of serum leptin and TSH in the vitamin D deficiency group and the vitamin D deficiency group were higher than those in the vitamin D sufficient group,the differences were statistically significant(P<0.05).There were no significant differences in the levels of serum FT3,FT4,and TgAb with different vitamin D nutritional status(P>0.05).According to Pearson analysis,serum 25(OH)D3 was negatively correlated with leptin(r=-0.589,P<0.001);serum 25(OH)D3 was positively correlated with FT3,FT4,TgAb,and TPOAb(P<0.05),and TSH was negatively c
