摘  要:目的研究院内MDRAB相关耐药基因及分子流行病学特征,为抗菌药物合理使用及医院感染控制提供依据。方法对2019年2月至2019年10月间院内分离的15株MDRAB用多位点序列分型(MLST)鉴定菌株之间的亲缘关系,PCR法检测β-内酰胺类耐药基因、质粒介导的喹诺酮类耐药基因、氨基糖苷类耐药基因及消毒剂qacE△1基因。结果 MLST分析发现15株MDRAB有ST195(46.7%)、ST369(20%)、ST208(13.3%)、ST368(13.3%)和ST540(6.7%)5个序列类型,均属于克隆复合体CC92。D类β-内酰胺类相关耐药基因OXA-23酶基因、OXA-51酶基因和OXA-66酶基因都阳性,60%TEM-1酶基因阳性,未检测出B类β内酰胺酶基因。氨基糖甙类耐药基因检出率依次为ant(3″)-Ⅰ(100%)、armA(86.7%)、aac(6′)-Ⅰ(40%)和aac(3)-Ⅰ(26.7%)。100%MDRAB携带qacE△1基因。未检出质粒介导的喹诺酮类耐药基因。结论 ST195型、OXA-51型变异体OXA-66型酶基因和OXA-23型酶基因是该院分离的MDRAB最主要的流行株。Objective This study aims to investigate the MDRAB-related resistance genes and molecular epidemiological characteristics,providing guidance for rational use of antimicrobials and control of nosocomial infections.Methods Genetic relationships of the 15 strains of MDRAB isolated in the hospital between February 2019 and October 2019 were identified by multi-site sequence typing(MLST).β-lactam-related resistance genes,aminoglycoside resistance genes,plasmid-mediated quinolone resistance genes and disinfectant qacE△1 gene was detected by PCR.Results MLST analysis revealed that the 15 MDRAB strains had 5 sequence types:ST195(46.7%),ST369(20%),ST208(13.3%),ST368(13.3%),and ST540(6.7%),all belong to the cloning complex CC92.Class Dβ-lactam-related resistance genes:OXA-23 enzyme gene,OXA-51 enzyme gene,and OXA-66 enzyme gene were all positive,60% of TEM-1 enzyme genes were positive,and no class Bβ-lactamase genes were detected.The detection rates of aminoglycoside resistance genes were ant(3″)-Ⅰ (100%),armA(86.7%),aac(6’)-Ⅰ (40%)and aac(3)-Ⅰ (26.7%).100%MDRAB carried qacE△1 gene.No plasmid-mediated quinolone resistance genes were detected.Conclusion ST195 type,OXA-51 variant,OXA-66 enzyme gene and OXA-23 enzyme gene are the main epidemic strains of MDRAB isolated in this hospital.
